Copper deficiency myeloneuropathy with a history of malabsorption: a tale of two cases

Abstract: Acquired copper deficiency is rare but often seen among patients with intestinal malabsorption syndromes. Often times, these patients can develop myeloneuropathies with copper deficiency as an elusive culprit. With early diagnosis and appropriate supplementation, many symptoms, such as myeloneuropathy, can be reversed entirely. Checking copper levels should be included in the workup of myeloneuropathies to prevent irreversible damage and improving morbidity and mortality.

“…The exact prevalence of copper deficiency in active CeD is unclear, but conditions associated with malabsorption of macronutrients and gastrointestinal disease can impair copper uptake and contribute to suboptimal copper status. This deficiency can lead to anemia, thrombocytopenia, neutropenia, peripheral neuronal involvement, and myelopathy [206][207][208][209][210][211]. Disorders that affect the mucosal cells responsible for iron absorption, such as celiac disease, atrophic gastritis, Helicobacter pylori infection, and bariatric surgery, proton pump inhibitors 1…”

“…Copper deficiency-related myelopathy is similar to subacute combined degeneration of the spinal cord caused by vitamin B12 deficiency, leading to dorsal column deficits in cervical and/or thoracic cord. 12 MRI spine often shows increased T2 signals involving the dorsal column, and electrophysiological studies often indicate sensorimotor axonal polyneuropathy. 13 This is consistent with our study, seven patients (77.8%) had MRI findings which showed posterior column T2 hyperintensities, while most patients (70%) had abnormal nerve conduction studies.…”

“…Nutritional deficiencies related to the abovementioned causes include different vitamin or element deficiencies. It is worth mentioning that copper deficiency is rare to come across [1][2][3][4].…”
