Coping with chronic life-threatening illness: Psychosocial perspectives on end stage renal disease.

Abstract: The medical and psychosocial situation of end stage renal disease patients is described and the available research literature is critically reviewed under the following headings: mood-depression, suicide, rehabilitation, pain, marital and sexual functioning, quality of life, course of adjustment, compliance, dialysis unit environment, and sensory-cognitive deficits. “…Contrary to expectation, illness perception (IEQ) and level of cognitive depression (CDI) failed to explain significant variance in biochemical and dietician ratings of adherence. This is in contrast to many previous studies with usually younger cohorts (Binik, 1983;Kaplan De-Nour, 1994). The average age of the patients in this population was significantly higher than that in previous studies (49 per cent over 60 years).…”

“…Understandably, such disruptions are believed to compromise the quality of life in ESRD, and this issue has received considerable attention (Binik, 1983;Binik & Chowanec, 1986;Blagg, 1978;Cassileth et al, 1984;Devins, 1989, in press; R. G. Evans et al, 1985;Levy, 1981bLevy, , 1983. It has been argued that available renal replacement therapies differ significantly in the intrusions and demands that they impose upon patients (e.g., Kutner, Brogan, & Kutner, 1986).…”

“…One must also take into account the potential stress associated with deprivation of physical activity or social relations. Similar considerations might apply to the renal dialysis patient (see Binik, 1983;Levy, 1974). Should one, for example, engage in physical events which may undermine regulation of water intake, or participate in social gatherings which will disrupt diet maintenance?…”
