“…Overall agreement between the two expert endosonographers was 83 % and rated as almost perfect. This is somewhat better than described in previous reports where a moderate to substantial agreement was described 24 25 26 (kappa-values of 0.46, 0.65 and agreement of 68 %, respectively). Our high interobserver agreement might be explained by the fact that our two reviewers are highly trained and experienced endosonographers.…”

“…The Rosemont criteria provide the endoscopist with a solid diagnostic basis, although the classification appears complex because it requires calculation of main and side criteria. Interestingly, a Canadian study showed that the initial impression of an experienced endoscopist could have the same diagnostic value as the calculation of the Rosemont criteria and concluded that the calculation does not have to be performed in clinical practice [ 50 ]. Similar changes in the pancreatic parenchyma can be seen in the early stages of autoimmune pancreatitis, as described in the next section.…”

“… 46 Moderate to substantial inter-observer agreement can be achieved with these criteria. 47 , 48 The Rosemont criteria have been compared to conventional EUS criteria and found to be more stringent, and thus may reduce the false positive diagnostic rates in context of early chronic pancreatitis. 49 , 50 Defining the sensitivity of EUS criteria is challenging as a true gold standard comparator has been difficult to identify because pancreatic function tests, pancreatography, and histology may not recognize mild or moderate chronic pancreatitis.…”
