Controversial ultrasound findings in mid trimester pregnancy. Evidence based approach

Abstract: Mid trimester fetal anatomy scan is a fundamental part of routine antenatal care. Some U/S soft markers or controversial U/S signs are seen during the scan and create some confusion regarding their relation to fetal chromosomal abnormalities. Example of these signs: echogenic focus in the heart, echogenic bowel, renal pyelectasis, ventriculomegaly, polydactely, club foot, choroid plexus cyst, single umbilical artery. We are presenting an evidence based approach from the literature for management of these contr… Show more

“…In the study by Åhman et al [21], women diagnosed with soft markers in pregnancy stated that they would have preferred not to have known, or were hesitant about receiving, this information. The clinical value and management of soft markers are described in a number of articles [33][34][35]. These vary as to the type of follow-up care recommended, and how far soft markers should be considered significant or normal variants.…”

“…Dies wurde in einigen weiteren Studien bestätigt. Jedoch wurden diese an einem Hochrisikokollektiv durchgeführt und können daher nicht auf jede Schwangere übertragen werden [2]. Nyberg et al zeigten dann 2001, dass die isoliert auftretende Nierenbeckenerweiterung statistisch in keinem Zusammenhang mit einer Trisomie 21 zu sehen ist [16].…”

“…Controversial ultrasound findings need to be interpreted for its clinical relevance, as being advised by the recently published recommendation from our Working Group on Ultrasound in Obstetrics of the World Association of Perinatal Medicine (WAPM). 18 Doppler studies and three-dimensional sonography are frequently used for more details. When more advanced sonographic modalities are used, potential thermal effect that may lead to neuronal apoptosis has to be cautioned.…”
