CONTROLLED TRIAL of INTRAVENOUS AMINOPHYLLINE IN ACUTE CEREBRAL INFARCTION

Abstract: In a double-blind trial intravenous aminophylline was compared with placebo in 79 patients with acute cerebral infarction. Immediate improvement in the neurological evaluation score was significantly more frequent in patients receiving aminophylline (38 per cent) than in those on placebo (15 per cent); only patients with mild or moderately severe strokes responded to the injection. After 3 weeks, however, the treated patients did not fare significantly better than the controls in terms of neurological score an… Show more

“…The immediate improvement of the signs of local cerebral dysfunction, which can be observed after an intravenous bolus injection of aminophylline in some patients suffering from acute stroke (Schiemert 1951 ;Schmidt-Voight 1951 ;Mainzer 1953;Weihs 1954;Olivarius 1970;Geismar et al 1976), may be ascribed to the vasoconstrictory effect of theophylline on non-ischemic brain vasculature thus causing a n "inverse steal" phenomenon (Skinhsj & Paulson 1970).…”
