Wound management represents a continuous challenge for health systems worldwide, considering the growing incidence of wound-related comorbidities, such as diabetes, high blood pressure, obesity, and autoimmune diseases. In this context, hydrogels are considered viable options since they mimic the skin structure and promote autolysis and growth factor synthesis. Unfortunately, hydrogels are associated with several drawbacks, such as low mechanical strength and the potential toxicity of byproducts released after crosslinking reactions. To overcome these aspects, in this study new smart chitosan (CS)-based hydrogels were developed, using oxidized chitosan (oxCS) and hyaluronic acid (oxHA) as nontoxic crosslinkers. Three active product ingredients (APIs) (fusidic acid, allantoin, and coenzyme Q10), with proven biological effects, were considered for inclusion in the 3D polymer matrix. Therefore, six API-CS-oxCS/oxHA hydrogels were obtained. The presence of dynamic imino bonds in the hydrogels’ structure, which supports their self-healing and self-adapting properties, was confirmed by spectral methods. The hydrogels were characterized by SEM, swelling degree, pH, and the internal organization of the 3D matrix was studied by rheological behavior. Moreover, the cytotoxicity degree and the antimicrobial effects were also investigated. In conclusion, the developed API-CS-oxCS/oxHA hydrogels have real potential as smart materials in wound management, based on their self-healing and self-adapting properties, as well as on the benefits of APIs.

Coronary Artery Bypass Grafting (CABG) is a standard cardiac procedure with potential post-operative challenges, including wound management and self-care deficiencies. The role of nursing interventions in improving self-care among these patients remains crucial. This study aimed to determine the effects of nursing interventions on self-care and wound management among post-coronary artery bypass grafting patients. Employing a quasi-experimental design, the study conducted pre-post comparisons in a single group of 36 post-CABG patients. The participants, predominantly middle-aged with a mean age of 61, were profiled based on various demographic and health parameters. Nursing interventions were assessed using pre-defined scales and the Bates-Jensen Wound Assessment Tool to determine the pre- and post-intervention status of the individuals. The interventions revealed significant improvements in several care dimensions: Physical Care improved from a pre-intervention mean score of 51.61 (SD=1.82) to 61.97 (SD=2.55), witnessing a mean difference of -10.36 (95% CI: -10.79 to -9.93, p=0.000). Emotional care experienced a surge from 31.61 (SD=1.82) to 38.72 (SD=2.34), resulting in a mean difference of -7.11 (95% CI: -7.42 to -6.80, p=0.000). Spiritual care enhanced from 43.64 (SD=1.82) to 50.94 (SD=2.70) with a mean difference of -7.31 (95% CI: -7.78 to -6.84, p=0.000). Workplace Self-Care marked an increase from 36.61 (SD=1.82) to 45.06 (SD=2.03), having a mean difference of -8.44 (95% CI: -8.90 to -7.99, p=0.000). Balance scores bettered from 5.06 (SD=0.98) to 7.58 (SD=0.73), showing a mean difference of -2.53 (95% CI: -2.70 to -2.36, p=0.000). In contrast, Psychological Care saw a decline from 51.11 (SD=2.38) to 41.61 (SD=1.82) with a mean difference of 9.50 (95% CI: 9.06 to 9.94, p=0.000). The nursing interventions positively affected several self-care dimensions, aiding in the holistic recovery of post-CABG patients. However, a decrease in psychological care scores suggests the necessity for an integrated approach that equally addresses all care dimensions, including psychological care.

Investigating Microbial Interactions of Pseudomonas Aeruginosa and Staphylococcus Epidermidis in Mixed Biofilms: Implications for Chronic Biofilm Infections

Chronic wounds are sores that typically take longer than three months to heal as the key mechanisms involved are impaired. In tandem with an impaired immune system, biofilms are observed and found to impact wound repair processes. The microbial composition of wounds is diverse, and the bacterial bioburden influences wound outcome. Commensals and environmental microbes occupy these wounds, however, only pathogenpathogen interactions have been explored thoroughly whereas commensals like Staphylococcus epidermidis (S. epidermidis) have only recently garnered interest. Established a mixed biofilm of S. epidermidis and common chronic wound pathogen Pseudomonas aeruginosa (P. aeruginosa) to investigate commensal-pathogen interspecies interactions, specifically on the antimicrobial resistance and virulence factor production of P. aeruginosa. Attenuated susceptibility of P. aeruginosa to anti-pseudomonal antibiotic ciprofloxacin was documented when co-cultured together with S. epidermidis in mixed biofilm. Proteins and extracellular metabolites within the supernatant of S. epidermidis demonstrated similar attenuation in antibiotic resistance of P. aeruginosa, although the active biological compounds remain unknown. Virulence factors of P. aeruginosa, pyocyanin and pyoverdine, were analyzed and demonstrated varying effects in dual-mixed biofilms with S. epidermidis based on iron availability. However, follow-up analyses revealed pyoverdine was not a major factor in contributing to the decrease in S. epidermidis viability in co-cultured biofilms. The research completed advances the knowledge of microbial interactions within chronic wounds, particularly the interactions between S. epidermidis and P. aeruginosa, and contributes to our understanding of how antibiotic resistance is affected in polymicrobial biofilms.
