Contribution of cell block obtained by endobronchial ultrasound-guided transbronchial needle aspiration in the diagnosis of malignant diseases and sarcoidosis

Abstract: Aim:Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a safe and minimally invasive procedure that can be performed in outpatient settings. Several studies have demonstrated the usefulness of EBUS-TBNA in the diagnosis of sarcoidosis and malignant diseases. This study focused on the role of cell block (CB) analysis in determining the diagnostic yield of EBUS-TBNA in malignant diseases and sarcoidosis.Materials and Methods:The study was conducted at a training and research hospital… Show more

“…In a similar study,[ 21 ] the contribution of CBs to the diagnostic yield of EBUS-TBNA for cases of malignancy and sarcoidosis was assessed. When cytological examination of smears was not diagnostic, but CB found granulomatous inflammation or malignancy, it was defined as “contribution to diagnosis by CB.” When cytological examination of smears was reported as only malignancy, but CB or immunohistochemical staining (IHCS) reported the subtype of cancer, it was defined as “contribution to subclassification by CB” [ Table 6 ].…”

“…In our study, the primary advantage of adding cell blocks to the diagnostic flow was a marked increase in the definite subtyping of malignancy which confirms the findings of other studies. [ 14 15 16 ] This was primarily an effect of the ease with which IHC could be applied to cell blocks. The extent of increased information due to subtyping is greater than reported by some studies,[ 15 16 ] possibly because many of the tumors were difficult to classify due to the poor differentiation, but is similar to others.…”

[ 20 ] The increase in the proper subtype given is essential for selecting further treatment and investigations, especially for Gemcitabine therapy, radiotherapy and selection of molecular markers for targeted therapy.…”

“…EBUS-TBNA provides a higher diagnostic yield than conventional bronchoscopy in cases of lymphoma, lung cancer, sarcoidosis, and TB. [23242526272829] It can reduce the number of punctures and improve the positivity rate. TBNA is performed under real-time monitoring of ultrasound images, which effectively avoids damage to the surrounding large blood vessels and improves the safety and accuracy.…”
