Contrast-Enhanced Ultrasound of the Liver: Optimizing Technique and Clinical Applications

Abstract: Proper technique and optimization of contrast-enhanced ultrasound require a balance between maintaining the integrity of the microbubble contrast agent and preserving the ultrasound signal. Established and emerging applications in the liver include diagnosis of focal lesions, aiding ultrasound-guided intervention, monitoring of therapy, and aiding surgical management.

“…SonoVue® (Bracco SpA, Milan, Italy) is the most commonly adopted CA in Europe and recently, has also been approved for hepatic perfusion imaging in adults and pediatric patients by the FDA in 2016 [9, 10]. Definity®/Luminity® (Lantheus Medical, Billerica, MA, USA) is commonly used in the US but has also recently been licensed in Canada and Australia [9, 10]. Sonazoid TM (Daiichi-Sankyo, GE, Tokyo, Japan) is mostly used in Japan and South Korea [9, 10].…”

It contains a hydrogenated egg phosphatidylserine on its outer shell which is phagocytosed by Kupffer cells of the liver while maintaining its echogenicity; this not only enables observation of hepatic perfusion but also the Kupffer cells [10].…”

“…Several brands are approved by the Food and Drug Administration in the United States, and they utilize similar microbubble technology. Of the agents approved, contrast with a sulfur hexafluoride core (Lumason ® , Bracco Diagnostics Inc., Monroe Township, NJ, USA) is widely used in the United States, Europe, and China [13] and at our own institution. CEUS has several unique features in comparison with CT or MRI with contrast.…”

“…CEUS has several unique features in comparison with CT or MRI with contrast. The contrast agent used with US is made of microbubbles that are smaller than a red blood cell, so that they do not cross the endothelium but instead remain in the intravascular space [13,14]. The microbubbles burst after several minutes of imaging, with byproducts exhaled through the pulmonary system [13].…”

“…The main methods to assess both liver quality and the pathological changes include ultrasound, positron emission tomography (PET), magnetic resonance imaging (MRI), computed tomography (CT), biochemical blood analysis, morphological and immunohistochemical analysis of biopsy specimens, and clearance tests using indocyanine, as well as various radioactive isotopes. [1][2][3] Ultrasound, PET, MRI, and CT are quite informative, but they only provide information about the liver tissue at the level of the *Address all correspondence to Daria Kuznetsova, E-mail: daria.s.kuznetsova@gmail.com whole organ, not allowing resolution at a cellular level. Biochemical blood analysis is among the simplest methods for studying liver function.…”
