Contrast-enhanced ultrasound (CEUS) appearances of an adrenal phaeochromocytoma in a child with Von Hippel-Lindau disease

Abstract: A phaeochromocytoma is a rare catecholamine-secreting tumour arising from the chromaffin cells. We describe a case of a child with Von Hippel-Lindau disease, with an adrenal phaeochromocytoma who presented with severe dilated cardiomyopathy driven by secondary hypertension. Contrast-enhanced ultrasound findings are described and compared with both magnetic resonance imaging and computed tomography imaging.

“…Dynamic CEUS using time-intensity curve analysis has been deployed in the investigation of adrenal gland tumors without clear differentiation [120,122]. CEUS may demonstrate characteristic hypervascularity of some adrenal gland tumors, e. g., pheochromocytoma, which typically also have necrotic regions with no contrast enhancement [121,123,124].…”

“…No data regarding the use of CEUS in children for further characterization of adrenal gland tumours is available. Isolated case reports documenting adrenal lesions in children are available [114,115]. Evaluation of trauma to the adrenal gland and differentiation of haematoma from underling adrenal gland tumour may be helpful [77].…”

“…68 The application of CEUS to trauma of the adrenal gland in children shows good correlation with CT imaging. 69 Nevertheless, it is characterized by limited accuracy in differentiating benign and malignant adrenal lesions in adults.…”
