Contrast-enhanced sonographic findings in a case of hepatobiliary cystadenoma with intracystic bleeding

Abstract: Intracystic slow bleeding is very difficult to diagnose using conventional sonography; consequently, a new sonographic technique has been sought. We present a histologically proven hepatobiliary cystadenoma with intracystic bleeding in which contrast-enhanced sonography (CEUS) showed microbubbles oozing from the cyst wall into the cystic cavity 10 minutes after intravenous injection of contrast medium. CEUS is an important diagnostic tool for diagnosing liver tumors, but the CEUS finding of slow intracystic bl… Show more

“…Though it is a rare complication, intracystic bleeding may occur [7]. Due to the secreted mucin, that can mimic intracystic bleeding, the diagnosis of a real intracystic bleeding is very difficult to perform using US alone.…”

“…Due to the secreted mucin, that can mimic intracystic bleeding, the diagnosis of a real intracystic bleeding is very difficult to perform using US alone. According to Naganuma H et al [7] findings, intracystic bleeding can be diagnosed through the use of CEUS. Unlike a regular examination, the evaluation of the cyst in this case should be performed after 5 minutes from the moment of the intravenous injection of the contrast agent.…”

“…Contrast-enhanced US imaging may play an important role in making a correct diagnosis of a hemorrhagic cyst since it demonstrates the avascularity of the visualized intracystic structures (11) and microbubbles oozing from the cyst wall into the cystic cavity (12). It should be noted that intracystic bleeding can occur in cystic liver tumors (12)(13)(14).…”

“…If elevated CA19-9 and CEA are detected in the cystic fluid or the epithelial lining, it is helpful to the diagnosis of hepatobiliary cystadenoma[11]. Since it is believed to be premalignant, complete resection is the best treatment[6],[12]; meanwhile, it is very important to follow up patients with hepatobiliary cystadenoma.…”
