Contralateral Tension Pneumothorax during One Lung Ventilation by a Univent® Tube

Abstract: Tension pneumothorax during one-lung ventilation (OLV) is a rare but life-threatening complication. A 79-year-old male patient who was diagnosed with lung cancer underwent Univent ® Tube (Fuji Systems Corporation, Tokyo)intubation for left upper lobectomy. Two hours after the initiation of OLV, the patient could not tolerate it. Thus, oneand two-lung ventilation were alternatively applied to continue the operation. After the operation, an emergent chest radiograph was taken, and pneumothorax was found at the r… Show more

“…The mediastinal shift caused by the tension pneumothorax puts pressure on the contralateral lung and vena cava, which can result in respiratory insufficiency, cardiovascular compromise, and death if not treated promptly (Sahota & Sayad, 2022). Mortality of patients with tension pneumothorax can increase from 3-7% when treated immediately to 91% if the treatment is delayed (No, Moon, & Kim, 2012;Roberts et al, 2015). Therefore, early recognition of clinical symptoms and appropriate management in the emergency department will reduce mortality in tension pneumothorax.…”

“…A pneumothorax in a critically ill patient who requires mechanical ventilation remains a serious condition [1][2][3]. It should be considered a medical emergency requiring a high index of suspicion, prompt recognition, and intervention.…”
