Background: In the European Union, 300 000 newborn babies are born prematurely every year.Their care is ensured in Neonatal Intensive Care Units (NICU) where vital signs are constantly monitored. In addition, other descriptors such as motion, facial and vocal activities have been shown to be essential to assess neurobehavioral development.Aim: In the scope of the European project Digi-NewB, we aimed to develop and evaluate a new audio-video device designed to non-invasively acquire multi-modal data (audio, video and thermal images) , while fitting the wide variety of bedding environment in NICU.Methods: Firstly, a multimodal system and associated software and guidelines to collect data in neonatal intensive care unit were proposed. Secondly, methods for post-evaluation of the acquisition phase were developed, including the study of clinician feedback and a qualitative analysis of the data. Results:The deployment of 19 acquisition devices in six French hospitals allowed to record more than 500 newborns of different gestational and postmenstrual ages. After the acquisition phase, clinical feedback was mostly positive. In addition, quality of more than 300 recordings was inspected and showed that 77% of the data is exploitable. In depth , the percentage of sole presence of the newborn was estimated at 62% within recordings.Conclusions: This study demonstrates that audio-video acquisitions are feasible on a large scale in real life in NICU. The experience also allowed us to make a clear observation of the requirements and challenges that will have to be overcome in order to set up audio-video monitoring methods.

Epilepsy is a serious neurological disorder characterized by abnormal, recurrent, and synchronous discharges in the brain. Long-term recurrent seizure attacks can cause serious damage to brain function, which is usually observed in patients with temporal lobe epilepsy. Controlling seizure attacks is vital for the treatment and prognosis of epilepsy. Animal models, such as the kindling model, which was the most widely used model in the past, allow the understanding of the potential epileptogenic mechanisms and selection of antiepileptic drugs. In recent years, various animal models of epilepsy have been established to mimic different seizure types, without clear merits and demerits. Accordingly, this review provides a summary of the views mentioned above, aiming to provide a reference for animal model selection.

Astrocyte reactivity in a mouse model of SCN8A epileptic encephalopathy

Objective SCN8A epileptic encephalopathy is caused predominantly by de novo gain‐of‐function mutations in the voltage‐gated sodium channel Nav1.6. The disorder is characterized by early onset of seizures and developmental delay. Most patients with SCN8A epileptic encephalopathy are refractory to current anti‐seizure medications. Previous studies determining the mechanisms of this disease have focused on neuronal dysfunction as Nav1.6 is expressed by neurons and plays a critical role in controlling neuronal excitability. However, glial dysfunction has been implicated in epilepsy and alterations in glial physiology could contribute to the pathology of SCN8A encephalopathy. In the current study, we examined alterations in astrocyte and microglia physiology in the development of seizures in a mouse model of SCN8A epileptic encephalopathy. Methods Using immunohistochemistry, we assessed microglia and astrocyte reactivity before and after the onset of spontaneous seizures. Expression of glutamine synthetase and Nav1.6, and Kir4.1 channel currents were assessed in astrocytes in wild‐type (WT) mice and mice carrying the N1768D SCN8A mutation (D/+). Results Astrocytes in spontaneously seizing D/+ mice become reactive and increase expression of glial fibrillary acidic protein (GFAP), a marker of astrocyte reactivity. These same astrocytes exhibited reduced barium‐sensitive Kir4.1 currents compared to age‐matched WT mice and decreased expression of glutamine synthetase. These alterations were only observed in spontaneously seizing mice and not before the onset of seizures. In contrast, microglial morphology remained unchanged before and after the onset of seizures. Significance Astrocytes, but not microglia, become reactive only after the onset of spontaneous seizures in a mouse model of SCN8A encephalopathy. Reactive astrocytes have reduced Kir4.1‐mediated currents, which would impair their ability to buffer potassium. Reduced expression of glutamine synthetase would modulate the availability of neurotransmitters to excitatory and inhibitory neurons. These deficits in potassium and glutamate handling by astrocytes could exacerbate seizures in SCN8A epileptic encephalopathy. Targeting astrocytes may provide a new therapeutic approach to seizure suppression.
