Continuous Quality Improvement and Comprehensive Primary Health Care: A Systems Framework to Improve Service Quality and Health Outcomes

Abstract: Continuous quality improvement (CQI) processes for improving clinical care and health outcomes have been implemented by primary health-care services, with resultant health-care impacts. But only 10–20% of gain in health outcomes is contributed by health-care services; a much larger share is determined by social and cultural factors. This perspective paper argues that health care and health outcomes can be enhanced through applying CQI as a systems approach to comprehensive primary health care. Referring to the… Show more

“…The application of continuous quality improvement (CQI) cycles using routinely collected data is a way to engage clinic staff in understanding their own data to seek to motivate behavior change. We [ 50 , 53 ] and others [ 54 ] have shown the effectiveness of this type of approach in achieving health system improvement. The approach is very applicable in this project where TB data are routinely collected and reported, but often not reflected upon, especially not in real time.…”

“…Where appropriate, recommendations arising from these CQI workshops to address challenges in policy implementation (e.g., staffing allocations, access to transport for outreach services) will be communicated to the District Health Authority. The approach will draw on lessons learned in previous research using CQI cycles in primary care to improve guideline adherence [ 48 – 50 ].…”

“…CQI processes aim to engage staff in understanding and responding to their own data and can be a powerful tool to motivate change in primary care settings 10. We previously showed incremental improvements in clinical care targets achieved in RHD management using CQI 2.…”

“…This is facilitated through individual and workshop training, education and advocacy provided by a dedicated project support team and PHN staff, along with the provision of a user manual and other relevant resources. The project team works with general practices using a stepwise approach to continuous and progressive quality improvement on a consistent set of measurable yet practical metrics adapted and inspired from several internationally proven quality improvement and system redesign methodologies (Geonnotti et al 2015;Project Management Group 2017; Australian and New Zealand College of Anaesthetists (ANZCA) 2018; McCalman et al 2018;Fig. 1).…”
