Continuous Infusion of Beta-Lactam Antibiotics in Severe Sepsis: A Multicenter Double-Blind, Randomized Controlled Trial

Abstract: Continuous administration of beta-lactam antibiotics achieved higher plasma antibiotic concentrations than intermittent administration with improvement in clinical cure. This study provides a strong rationale for further multicenter trials with sufficient power to identify differences in patient-centered endpoints.

“…We referred to 4 RCT reports 72, 90, 91, 92. Among these studies, no significant differences were observed between the respective study groups with respect to 90‐day mortality rate (odds ratio: 0.94; 95% confidence interval [CI]: 0.69–1.28, P  =   0.68), ICU mortality rate (odds ratio: 0.79; 95% CI: 0.59–1.06, P  =   0.11), in‐hospital mortality rate (odds ratio: 0.78; 95% CI: 0.59–1.03, P  =   0.08), or target serum drug concentration achievement rate (odds ratio: 1.88; 95% CI: 0.89–3.98, P  =   0.10).…”

“…3 The NHMRC has already demonstrated a willingness to fund clinician-initiated RCTs, as shown by the success of networks such as the Australian and New Zealand Intensive Care Society Clinical Trials Group. [4][5][6][7] The impact of their successfully completed trials on practice change, co st savings and improved patient outcomes has not been formally measured but is likely to be significant.…”

“…Continuous infusion (CI) could also be used as alternative administration for b-lactams in lung infections. [24][25][26]40,41 In patients with moderate to severe renal failure, a reduction in drug clearance may help in the achievement of PK/PD targets with usual dosing regimens. 24 …”
