ObjectivesPresbyphagia refers to age-related changes in the swallowing mechanism (e.g., reduced skeletal muscle strength that decreases bolus control). If left untreated, these changes can lead to dysphagia, which refers to impaired swallowing (e.g., coughing or choking when eating). Given that swallowing difficulties are common among older adults that they make up the fastest growing age group globally, the need for interventions to address presbyphagia is gaining urgency. To begin to address this need, we conducted a scoping review to analyze music therapy research aimed at enhancing swallowing function. The objective was to identify key intervention characteristics and propose clinical implications for treating presbyphagia using music therapy.MethodsThis review followed the methodological frameworks outlined by Arksey and O’Malley and Levac et al. and used the Preferred Reporting Items for Systematic Reviews and Meta-Analysis for Scoping Reviews for analysis and reporting. Four electronic databases (i.e., ProQuest, PubMed, RISS, Web of Science) were searched for quantitative and qualitative studies in English or Korean that used music-based interventions to address swallowing function in older adults. Content analysis was conducted to identify and compare the main features of music interventions for swallowing difficulties among older adults.ResultsTen articles were identified and analyzed. It was found that three core components–respiration, vocalization, and singing–were employed to enhance swallowing function in populations with neurological impairments, dementia, or head and neck cancer. Notably, actions closely linked to swallowing function, such as laryngeal elevation and oral movements, were utilized therapeutically to speak or sing. Based on these characteristics, clinical implications are proposed to address presbyphagia.ConclusionSinging entails a systematic and focused incorporation of stepwise activities that can be used to address swallowing disorders. In this context, critical clinical implications that music therapists should consider when treating individuals with presbyphagia include warmup breathing, vocalizing targeting laryngeal control, and singing targeting oral motor control. This review can contribute to the expansion of music therapy with older adults and the advancement of music therapy techniques.

Objectives: Aging-related changes in swallowing functions have rapidly become prevalent among the elderly. Self-reported questionnaires are often conducted to observe how people perceive their swallowing functions, and to report the swallowing-associated qualityof-life. This study was conducted to demonstrate the use of Rasch analysis for validating the pre-developed self-reported questionnaire to assess swallowing functions for community-dwelling older adults. Methods: A total of 479 older adults (men= 238, mean age± SD= 75.6 ± 7.17 years) were enrolled. Twenty-six items with respect to swallowing functions were completed by the participants. Each item investigated symptom frequency or severity on a 5-point Likert scale. A Rasch analysis using the WINSTEP ® program was undertaken to establish validity and reliability of the questionnaire. Results: From the item fit statistics analysis, 11 out of 26 items were shown to be misfits to the model. Thus, the 15-item Brief Inventory of Swallowing Ability (BISA-15) was derived and demonstrated a unidimensional scale based on the dimensionality and item fit statistics. The overall item reliability was excellent. The person reliability was lower than the required threshold. The rating scales needed to collapse into three categories both in frequency and severity (i.e., never, sometimes/moderate, often/severe). Conclusion: The BISA-15 can be utilized to detect degree of aberrations in swallowing that transit from normal to subclinical stage in older adults. Future studies are warranted to further clarify the utility of the BISA-15 in a wide range of age in the elderly as well as in the various clinical populations.

Since swallowing function deteriorates with aging, it is crucial to examine the elderly's perception of their swallowing function and quality of life-swallowing. The purpose of this study was to establish the concurrent validity of the Swallowing Monitoring & Assessment Protocol (SMAP) and the Korean-Dysphagia Handicap Index (K-DHI). Methods: The results of the one-on-one survey via paper-pencil or mobile format presented to 91 older adults (men 20, women 71; mean age, 78 ± 6.47 years) among 579 participants recruited from the 16 senior welfare or senior community centers were analyzed. Individual responses were converted into scores and Pearson correlation and Spearman rank order statistical analyses were done via the IBM SPSS Statistics version 25. Results: There was a strong correlation (r= .825, p < .01) between the overall scores of the SMAP and the K-DHI. Analysis of rank ordered data of response frequency of the 13 items with similar contents in two evaluation tools also revealed a moderate correlation (r= .655, p < .05). Items related with 'dry mouth' ranked as the first complaint in both evaluation tools. Also, 'choking' and 'coughing' on liquids or water was ranked third (SMAP) and second (K-DHI) complaints, respectively, in both evaluation tools. Conclusion: It should be recognized that some swallowing problems observed in older adults may be a consequence of 'ageing' rather than features of 'disorder' , per se.
