Contemporary adjuvant polymethyl methacrylate cementation optimally limits recurrence in primary giant cell tumor of bone patients compared to bone grafting: a systematic review and meta-analysis

Abstract: BackgroundReports of recurrence following restructuring of primary giant cell tumor (GCT) defects using polymethyl methacrylate (PMMA) bone cementation or allogeneic bone graft with and without adjuvants for intralesional curettage vary widely. Systematic review and meta-analysis were conducted to investigate efficacy of PMMA bone cementation and allogeneic bone grafting following intralesional curettage for GCT.MethodsMedline, EMBASE, Google Scholar, and Cochrane databases were searched for studies reporting … Show more

“…Intralesional curettage + bone grafting alone has some controversies compared to curettage + cementing and also adding adjuvant therapies. In the systematic review of Zuo et al (22) published in 2013, local recurrence rate was higher in curettage + bone grafting than in curettage + cementing patients. Also, curettage + grafting + adjuvant treatment was found with a higher recurrence rate than curettage + cementing + adjuvant patients in the same systematic review.…”

“…Traditionally, bone graft or bone cement is used in humans. 1,2,13,24 In the present case, and for several reasons, PMMA bone cement was chosen. The exothermic reaction, which occurs as the cement polymerizes, releases heat which may destroy any tumour cells remaining on the borders of the lesion after curettage and coblation, particularly under the tibial plateau where curettage was done with caution.…”

“…Local tumour recurrence was found to be minimal in human patients treated with PMMA cement as compared with bone grafting. 13,24 The long-term prognosis regarding joint function after subchondral bone replacement by cement remains unknown. Curettage may have a deleterious effect on articular cartilage when it is performed adjacent to a joint.…”
