Routine pre-employment chest radiography is one of the examinations performed to assess the medical fitness of prospective employees in many establishments around the world. It is also used to ensure that prospective employees can perform their jobs safely without placing co-workers and others at potential health risk by ruling out the existence of chronic infectious pulmonary diseases like tuberculosis. Despite these focused goals, there are concerns over poor positive result yield, associated unnecessary exposure to ionizing radiation and wastage of economic resources linked to pre-employment chest xray. This study aimed to audit the reports of pre-employment chest radiographs, documented associated radiological findings and subsequently examined the clinical relevance or otherwise of this practice in the locality under study. The records of 725 patients, who had underwent pre-employment chest x-rays between January 2014 and December 2016 in three foremost teaching hospitals in South-western Nigeria, were retrospectively reviewed. All findings were documented including abnormalities 'flagged' as clinically-significant by the radiologists' reports irrespective of the severity or the effect of the abnormality on the employment outcome of the individual. The results show that majority of the patients referred for preemployment chest x-ray presented with normal radiological findings (n = 687, 94.8%). The significant abnormal findings (n = 38, 5.2%) were chronic cardiovascular disorder (n = 15, 2.0%), followed by chronic lung disease (n = 18, 2.5%) and finally skeletal abnormality (n = 5, 0.7%). Individuals 25 years and below presented with no abnormal findings while abnormal findings are found in subjects in the age range of 26-30 years and above. This audit has shown that chest radiography has little or no clinical relevance in routine pre-employment medical screening of physically healthy and asymptomatic prospective employees. Therefore, chest radiography should be restricted to individuals with clinical findings suggestive of cardio-thoracic disorders during pre-employment medical screening.

Tuberculosis is a major health problem throughout the world causing large number of deaths, more than that of any other single infectious disease. Accurate and early diagnosis of tuberculosis is important for its effective management. The continued development of affordable, sensitive, and specific diagnostic tools will be required for use in resource-poor settings, where the incidence of disease is the highest like Ethiopia. This paper was done to give concise review on various convectional and advanced diagnostic techniques of human tuberculosis, focused on diagnostic techniques used in clinical and research setting in Ethiopia.

Abstrak: Tuberkulosis (TB) masih menjadi masalah kesehatan dunia yang utama sampai sekarang. Menurut WHO, Indonesia masih menjadi negara dengan peringkat keempat sebagai penyumbang TB terbesar dengan 400-500 ribu kasus. WHO melaporkan bahwa ada sekitar 327.000 kasus baru TB pada anak dengan usia <15 tahun di seluruh dunia, dan sekitar 65.000 anak meninggal karena TB setiap tahun. Dalam mendiagnosis TB anak perlu dilakukan pemeriksaan uji tuberkulin dan foto toraks. Tujuan penelitian ini adalah untuk mengetahui hubungan gambaran foto toraks dan uji tuberkulin pada anak dengan diagnosis tuberkulosis paru. Penelitian ini dilakukan di RSUP Prof. Dr. R. D. Kandou Manado periode Januari 2012 – Desember 2012. Penelitian ini bersifat analitik observasional menggunakan desain cross sectional dengan data retrospektif. Pengambilan sampel pada penelitian ini menggunakan teknik total sampling. Data yang dikumpulkan diolah dengan menggunakan uji korelasi Kendall’s tau-b. Dari hasil analisa data didapatkan nilai r= -0,408 dengan p= 0,046 (p<0,05). Nilai ini berarti bahwa ada hubungan negatif sedang antara gambaran foto toraks dan uji tuberkulin. Dengan demikian, pemeriksaan foto toraks dan uji tuberkulin harus dilakukan dalam membantu menegakkan diagnosis tuberkulosis paru pada anak. According to WHO, Indonesia has ranked as the fourth largest contributor TB with 400-500 cases. WHO reports that there are approximately 327,000 new cases of TB in children aged <15 years old all over the world, and about 65,000 children die from TB every year. In diagnosing TB in children, tuberculin test and chest x-ray are necessary. The purpose of this research is to determine the relationship between a chest x-ray picture and tuberculin test in children diagnosed with pulmonary tuberculosis. This research was conducted in RSUP Prof. Dr. R. D. Kandou Manado in the period of January 2012 – December 2012. This was an observational analytic research using cross-sectional design with retrospective data. There were 25 children observed in this research. Analysis shows a negative correlation between radiographic picture and tuberculin test (r= -0.408; p= 0.046). Accordingly, chest x-ray examination and tuberculin test should be performed to help justify the diagnosis of pulmonary tuberculosis in children. Abstract: It is concluded that other ancillary tests like sputum examination and/or, where available, Interferon and ESAT tests should be carried out before chest radiograph is requested in subjects with positive Mantoux reaction.
