Contact-Lens-Associated Purpureocillium Keratitis: Risk Factors, Microbiologic Characteristics, Clinical Course, and Outcomes

Abstract: Purpureocillium keratitis can have devastating consequences to visual function and even lead to enucleation. Physicians should make every effort to arrive at an earlier microbiological diagnosis, as this is associated with better outcomes and less need for surgical intervention. The first line use of voriconazole is recommended, and steroid use should be avoided, as their previous use is associated with worse visual outcomes.

“…29 P. lilacinum is also one of the important species of fungal keratitis that is known to be resistant to polyenes and sensitive only to VRCZ in most of the strains. 13,30 The findings of all previous studies suggest that there is a strong need to create a new antifungal drug for potential use as a first-line topical drug.…”

“…28,29 All three cases in our cohort were in the Cairns and Hinterland HHS in far north Queensland. 31 One retrospective case series also appears to suggest that Purpureocillium keratitis is more prevalent in sub-tropical Queensland. 30 A discussion of the ecological niche of Purpureocillium is beyond the scope of this article, but climate may be but one factor influencing the incidence of Purpureocillium keratitis; a study of contact lens-associated Purpureocillium keratitis in south Florida, which has a similar Köppen climate classification to far north Queensland, found 28 cases between 2007 and 2014.…”

“…Most clinical manifestations are associated with ocular and cutaneous/subcutaneous infections as described by Borba and Brito [9,63,64,65,66,67,68,69,70,71,72,73,74]. More recently, an onychomycosis [75] and a rare case of nasal perforation [76] were published.…”
