Consumo de medicamentos para el tratamiento de la demencia en la Comunidad Autónoma Vasca durante el periodo 2006-2011

Objectives: The only recommended pharmacological treatments for specific dementias are donepezil, galantamine, rivastigmine, and memantine (recommended drugs, RD). However, other drugs without recommendations (not recommended drugs, NRD) are often used to treat patients with cognitive impairment (CI) in Argentina. The INSSJyP is the largest health insurance in Argentina. The objective of this study is to analyze the prescription pattern, cost, and implications of NRD used for the treatment of CI in the INSSJyP. Materials: This is a retrospective, population-based study of the INSSJyP outpatients' prescriptions database for drugs usually prescribed for CI during 2015. These data were compared with the same database in 2009. The number of "prescriptions" always refers to dispensed packages. Results: A total of 3 255 438 packages of drugs usually indicated for CI were prescribed during 2015: 1 912 476 packages of RD (59%) and 1 342 962 packages of NRD (41%).Comparing the results with those obtained in 2009, there is a 148% gross increase in the prescription of both RD and NRD for CI, although the rates/1000 affiliates/year show a lesser rise for NRD (70.1%) compared to RD (103.9 %).The expenditure on CI drugs prescribed during 2015 was 77 million USD. NRD cost represented approximately 20 million USD. Conclusion: Inappropriate drug use increases health costs in developing countries.We found a high number of patients with a probable diagnosis of CI treated with NRD. It is extremely relevant that all the healthcare professionals can update their knowledge and modify behavioral insights about appropriate prescription for specific dementias.

Background An updated time-trend analysis of anti-dementia drugs (ADDs) is lacking. The aim of this study is to assess the incident rate (IR) of ADD in individuals with dementia using real-world data. Setting Primary care data (country/database) from the UK/CPRD-GOLD (2007–20), Spain/SIDIAP (2010–20) and the Netherlands/IPCI (2008–20), standardised to a common data model. Methods Cohort study. Participants: dementia patients ≥40 years old with ≥1 year of previous data. Follow-up: until the end of the study period, transfer out of the catchment area, death or incident prescription of rivastigmine, galantamine, donepezil or memantine. Other variables: age/sex, type of dementia, comorbidities. Statistics: overall and yearly age/sex IR, with 95% confidence interval, per 100,000 person-years (IR per 105 PY (95%CI)). Results We identified a total of (incident anti-dementia users/dementia patients) 41,024/110,642 in UK/CPRD-GOLD, 51,667/134,927 in Spain/SIDIAP and 2,088/17,559 in the Netherlands/IPCI. In the UK, IR (per 105 PY (95%CI)) of ADD decreased from 2007 (30,829 (28,891–32,862)) to 2010 (17,793 (17,083–18,524)), then increased up to 2019 (31,601 (30,483 to 32,749)) and decrease in 2020 (24,067 (23,021–25,148)). In Spain, IR (per 105 PY (95%CI)) of ADD decreased by 72% from 2010 (51,003 (49,199–52,855)) to 2020 (14,571 (14,109–15,043)). In the Netherlands, IR (per 105 PY (95%CI)) of ADD decreased by 77% from 2009 (21,151 (14,967–29,031)) to 2020 (4763 (4176–5409)). Subjects aged ≥65–79 years and men (in the UK and the Netherlands) initiated more frequently an ADD. Conclusions Treatment of dementia remains highly heterogeneous. Further consensus in the pharmacological management of patients living with dementia is urgently needed.

Las herramientas tecnológicas y didácticas en el aprendizaje de la educación han permitido el abordaje de los contenidos para responder de manera pertinente a las necesidades de aprendizaje estudiantil. Por esta razón, el presente trabajo tiene como objetivo determinar el manejo de las herramientas tecnológicas y didácticas en el aprendizaje de los valores cívicos en los docentes y estudiantes en la Unidad Educativa Guaranda 43 Portoviejo -Ecuador. Metodológicamente se define como una investigación descriptiva, exploratoria y bibliográfica. Para la discusión y análisis de los resultados se utilizaron los métodos teóricos de análisis, inducción y deducción. El instrumento utilizado fue una encuesta electrónica para recaudar la información. Participaron 44 estudiantes y 16 docentes. Los problemas que más se evidenciaron en la investigación fue la poca utilización y manejo de las herramientas tecnológicas por la situación socio económica de los estudiantes y la falta de conectividad en los sectores de vivienda. Los resultados evidenciaron que el manejo de las herramientas tecnológicas y didácticas en el aprendizaje de los valores cívicos presentan complicaciones, debido a la poca utilización de las herramientas tecnológicas educativas de parte de los docentes y estudiantes. Se concluye que, si bien los estudiantes no cuentan con el manejo de herramientas tecnológicas, existe gran aceptación sobre el uso de herramientas educativas básicas, conllevando a mejoramiento del aprendizaje y estrategias didácticas en el alumnado. Los docentes deben enfatizar diversas alternativas didácticas con la finalidad que los estudiantes sigan aplicando los valores cívicos a través de las herramientas tecnológicas educativas, independientemente del área de estudio.
