Scope: High-salt diets (HSDs) are widely considered to cause health problems such as gut microecological imbalances, constipation, and hypertension. This study explores how lactulose as a safe molecule can stimulate bodily responses to alleviate salt-sensitive hypertension by regulating the gut microbiotas of HSD-fed mice. Methods and results: After 4 weeks, the blood pressures of mice fed a high-salt plus lactulose diet (HSLD) are significantly lower than those of the HSD-fed mice. The HSD increases the abundances of Alistipes and Ruminococcaceae UCG 009 and reduced the abundance of Lactobacillus in the gut, while lactulose supplementation increases the abundances of Bifidobacterium, Alloprevotella, and Subdoligranulum. Fecal metabolic profiling shows significant increases in metabolites involved in ATP-binding cassette transporter pathways, and tryptophan metabolism is significantly reduced in the HSLD group compared with the HSD group. Lactulose maintains the intestinal microenvironmental health in the HSD-fed mice by improving glycolipid metabolism, decreasing the small intestinal interleukin-17a (IL-17a) and interleukin-22 (IL-22) mRNA levels and serum IL-17a and IL-22 levels, relieving constipation, increasing fecal sodium, and reducing intestinal permeability. Conclusion: Lactulose negates salt-sensitive hypertension. Regulating the gut microbiota is a potential treatment for salt-sensitive hypertension.

Introduction: Thyroid disease is extremely prevalent than heart diseases or diabetes. Hormones produced by the thyroid gland maintain the muscles, heart, brain, and other organs of the body. Improper performance of the thyroid gland leads to an overactive thyroid (hyperthyroidism) or underactive thyroid (hypothyroidism) that disturbs the metabolic system. The objective of the present study is to investigate the functional limitations of the patients under these two types of thyroid disorders. Methods: A cross-sectional study design is used to collect the data from thyroid disease patients in INMOL hospital Lahore, Pakistan from 23rd November 2020 to 20th January 2021. A total of 201 patients (98 hyperthyroid, 103 hypothyroid) are enrolled in this study. Data is analyzed through frequencies, percentages, and chi-square test of association by using SPSS (version 23). Results: Females over 30 years of ages, married, overweight with family sizes are less than four with a positive family history of brothers and sisters using iodized salt are more at risk of hypothyroidism and faced the problems like speedup in the metabolic system, weight gain, slow heartbeat, low blood pressure, weakness in muscles, constipation, coldness when others do not. Eventually it causes difficulties in routine activities, irregularities in the menstrual cycle and problems in pregnancy. Conclusion: By considering all these evidences, early diagnosis and precautionary steps are needed to avoid the prevalence of thyroid disease specifically in women.

Constipation can cause transient malfunction of the ventriculoperitoneal shunt (VPS). Patients with myelomeningocele or cerebral palsy are often diagnosed with hydrocephalus and constipation due to neurogenic bowel. These patients are more prone to VPS dysfunction, often requiring surgical revision. The authors report the case of a 6-year-old girl with a VPS that had been implanted due to hydrocephalus secondary to myelomeningocele. The patient was brought to the emergency department with intermittent headache, vomiting, constipation, and abdominal distension and pain. A CT scan revealed ventricular dilatation and radiography of the abdomen showed bowel loop distension. After a Fleet enema and digital maneuvers, her abdominal distension and symptoms improved. A CT scan obtained 24 hours later showed a reduction in ventricular size. The mechanism by which constipation can lead to VPS malfunction can be traced to indirect increases of intraabdominal pressure and direct obstruction of the catheter by distended intestinal loops. Treating constipation can restore the free circulation of the CSF and avoid surgical intervention. Careful neurological monitoring of these patients is essential, because some measures used to treat constipation can increase intracranial pressure. The objective of this report was to highlight constipation as a possible cause of transient VPS malfunction, thereby avoiding unnecessary surgical revisions, to which children with hydrocephalus are frequently submitted.
