“…135 Richer et al have recommended respectfully addressing reproductive status, anatomy, and sexual practices to gauge pregnancy risk, guide counselling, and provide the best care for these patients. 136 For example, it is important to consider that a transgender man may no longer have a uterus or may engage in sexual activities with individuals who do not have a penis and/or testes with reproductive potential. 136 Previously, the iPLEDGE program (United States of America) required clinicians to categorize patients as either: male, female of nonchildbearing potential, or female of childbearing potential and only the latter had to undergo strict pregnancy monitoring.…”

This classification system posed a significant dilemma for patients and clinicians who had to either categorize the patient according to their assigned sex at birth (and neglect the patient’s identity) or based on their gender identity (which did not respect prior iPLEDGE principles).…”

“…I read with interest the article by Richter and Kuritzky discussing considerations in treating severe acne with Isotretinoin in transgender men. 1 Although the authors state that " Trans men with native internal female reproductive organs have a risk of gestation that is difficult to quantify despite possibly finding themselves amenorrheic due to testosterone treatment," what is not specifically stated is that transgender men with a retained functional uterus and ovaries have become pregnant, both during and after testosterone therapy. 2 In a web-based survey from 2014, amongst 25 transgender men reporting using testosterone before pregnancy, 5 (20%) reported conceiving while still amenorrheic from testosterone use.…”
