Consideration of sense of coherence in a structured communication approach with stage IV lung cancer patients and their informal caregivers: a qualitative interview study

Abstract: Objective Salutogenetic aspects are valuable for consideration in patient-centred care of advanced oncological diseases with a limited life expectancy. The Milestone Communication Approach (MCA), involving physician-nurse tandems, addresses specific challenges and needs over the disease trajectory of patients with stage IV lung cancer and their informal caregivers. This study aims to explore patients' and informal caregivers' salutogenetic experiences with the MCA concept. Methods This qualitative study used f… Show more

“…Other research on work-related SOC has also shown that a strong SOC is correlated with health-related quality of working life [38], high levels of work engagement [39,40] and less work-related stress [41]. SOC has successfully been incorporated into other qualitative studies, mainly in healthcare settings [42][43][44]. A recent systematic review provided evidence that a SOC helps to protect against a depressive state, burnout and job dissatisfaction among nurses, and that it is important to build a healthy work environment to foster the SOC [45].…”

“…Getting timely and tailored information about the disease and what to expect in the future supports them in maintaining a sense of control [46]. Structured communication and having a responsive member of the health care team following up on patients assist in patient coping [13] and reducing uncertainty in a highly uncertain situation [47]. Interprofessional collaboration (notably physicians and nurses) and a longitudinal communication approach (including aspects of illness understanding, prognostic awareness, coping, and advance care planning) are key features for effectiveness in early palliative care according to cancer care guidelines [48].…”

“…This randomized trial was part of a larger multiphase mixed-method study focusing on the implementation and evaluation of the MCA concept in a thoracic oncology clinic [11]. Publications on the MCA concept development [10], implementation fidelity [12], and patient and caregiver experiences [13] are available. The present publication follows the recommendations of the Consolidated Standards of Reporting Trials (checklist available as supplemental online Table 1).…”
