ABS TRACT Objective: Sedentary lifestyle is a major risk factor for cardiovascular disease (CVS) and thrombotic complications. The effect of different types of exercise on platelet function has been the subject of a limited number of studies. Behçet's disease is a multisystemic disease characterized by recurrent oral and genital aphthous lesions and uveitis with unknown etiology. There is an unknown cause of the disease and hyperactivation of the procoagulant cascade and associated mortality due to arterial and venous thrombosis. Here, the effect of aerobic exercise on the bleeding profile and acute phase reactants in Behçet's patients was analyzed experimentally. Material and Methods: Twenty-four (n=24) patients with Behcet's disease were included in the study. Age, height, weight, sex, smoking, known systemic disease (such as diabetes, hypertension, thyroid) and vascular claudication were investigated. Those who didn't complete the protocole (n=5) were excluded from the study. Before and after moderate intensity aerobic exercise program (4 weeks, 3 times a week, 45 minutes per day) sedimentation (ESR), C-reactive protein (CRP), bleeding profile including prothrombin time (PT), prothrombin time percentage (PT%), activated prothrombin time (aPTT), platelet, collagen epinephrine, collagen adenosine diphosphate (ADP) and international normalized ratio (INR) were recorded. Results: Nineteen patients with Behçet's disease were enrolled. The mean age was 37.8±9.8 years. The mean body mass index (BMI) was 28.6±5.5 kg/m 2. Vascular claudication was present in 9 patients (47.4%), smoking in 1 patient (5.3%), and hypertension (10.5%) in 2 patients. While ESR (p=0.028) and collagen ADP (p=0.02) decreased after exercise; platelet count (p=0.005), aPTT (p=0.08), PT (p=0.022) and PT% (p=0.014) were increased. Conclusion: Increase in the duration of aggregation and decrease in inflammation after the exercise program supports that thrombosis can be prevented by exercise. We conclude that short term home-based aerobic exercise program has positive effect on homeostasis in patients with Behçet's disease.

Background Behçet’s syndrome is a type of systemic chronic vasculitis of unknown etiology, frequently characterized by recurrent oral and genital ulcers and uveitis. It is less commonly characterized by arthritis and skin, vascular, and gastrointestinal involvements. Behçet’s syndrome affects various sizes of vessels by perivascular infiltration and vasculitis. Unlike other classic types of vasculitis, Behçet’s syndrome patients can present with both arterial and venous involvement. Although vascular Behçet’s syndrome is found in only around 15% of Behçet’s syndrome patients, it is the major cause of morbidity and mortality among them. Furthermore, although deep venous thrombosis has high incidence in Behçet’s syndrome patients, pulmonary artery thrombosis is an uncommon complication. Combining the findings of this and previous case reports of pulmonary artery thrombosis in Behçet’s syndrome patients, we sought to determine the best treatment options for pulmonary artery thrombosis in Behçet’s syndrome patients. Case presentation We present the case of a 22-year-old Arabian male who was admitted to an emergency department with acute chest pain, dyspnea, and hemoptysis for 2 weeks. He gave a long history of recurrent oral and genital ulcers for the last 4 months but without seeking medical advice. Spiral computed tomography showed arterial filling defects with a pulmonary nodule for which the presence of a pulmonary artery aneurysm ruled out. The lung perfusion scan showed multiple pulmonary perfusion defects. After excluding common infectious diseases such as tuberculosis and brucellosis, a diagnosis of Behçet’s syndrome with pulmonary artery thrombosis was made. Steroids with enoxaparin were initiated. The patient was discharged later on prednisolone (tapering dose) with adalimumab and apixaban. He was on regular follow-up for the next 9 months. Conclusions Vascular involvement in Behçet’s syndrome is a major contributor to morbidity and mortality of Behçet’s syndrome patients. Consequently, early detection of vascular involvement has a major impact on the prognosis of patients with Behçet’s syndrome.

Ö ÖZ ZE ET T A Am ma aç ç: : Behçet hastalığı (BH)'nda vasküler tutulum genellikle derin ven trombozu (DVT) şeklinde olmaktadır. Bu çalışmada, DVT gelişen Behçet hastalarında immünsupresif tedaviyle kom-bine edilen farmakomekanik trombektomi (FMT) yönteminin uygulanabilirliğini ve bu yöntemin klinik iyileşme üzerine olan etkilerinin bir yıllık sonuçlarının sunulması amaçlanmıştır. G Ge er re eç ç v ve e Y Yö ön nt te em ml le er r: : Alt ekstremite akut veya subakut, ilk atak DVT tanısıyla kliniğimize yönlendirilen ve FMT uygulanan 27 Behçet hastası çalışmaya alındı. Hastalara, işlemden önce geçici vena kava in-ferior filtresi yerleştirildi ve işlemden sonra kateter yardımıyla 24 saat boyunca 1 mg/saat doku plazminojen aktivatörü (tPA) infüzyonu uygulandı. Takipte, hastaların fizik muayeneleri ve Villalta skorlaması yapıldı. Derin venöz sistem reflü süreleri 3. 6. ve 12. aylarda Doppler ultrasonografi (USG) ile değerlendirildi. B Bu ul lg gu ul la ar r: : Yirmi yedi (22 erkek ve 5 kadın) hastanın yaş ortalaması 28,8±5,8 yıl idi. BH'nın ortalama süresi 19,5±13,5 ay ve semptomların başlama süresi ortalama 6,4±4,0 gün bulundu. On yedi hastada femoropopliteal DVT ve 10 hastada iliofemoral DVT saptandı. Altı hastada bilateral femoropopliteal segment trombozu mevcuttu. FMT yöntemi, 33 alt ekstremiteye uy-gulandı ve işlemin ortalama süresi 104±23 dk idi. Yirmi (%74) hastada trombüsün tamamen uzaklaştırıldığı Grade III akım, 3 (%11)'ünde Grade II (%50-99) akım elde edildi. Altıncı ay takibinde iki hastada tekrarlayan DVT atağı gelişti. Villalta skoruna göre, on ikinci ay takibinde, altı hastada hafif posttrombotik sendrom (PTS) ve dört hastada ılımlı PTS kliniği saptandı. S So on nu uç ç: : İlk atak akut veya subakut DVT ile başvuran seçilmiş Behçet hastalarında, immünsupresif ve antikoagülan tedaviye ek olarak, FMT yöntemi ile trombüs yükünün azaltılmasının faydalı olabileceği düşünülmektedir. Ülkemizde BH'nın yaygın olması nedeni ile, bu hastalarda gelişen venöz tutulum tedavisini optimal düzeyde sağlayabilmek için daha kapsamlı ve karşılaştırmalı çalışmalara ihtiyaç vardır.
