Consensus statement on dose modifications of antidiabetic agents in patients with hepatic impairment

Abstract: Liver disease is an important cause of mortality in type 2 diabetes mellitus (T2DM). It is estimated that diabetes is the most common cause of liver disease in the United States. Virtually, entire spectrum of liver disease is seen in T2DM including abnormal liver enzymes, nonalcoholic fatty liver disease, cirrhosis, hepatocellular carcinoma, and acute liver failure. “…The liver plays a role in carbohydrate metabolism, gluconeogenesis and glycogenolysis. Impaired liver function ultimately contributes to an increased risk of hypoglycemia [25]. Based on a study conducted by Kagansky et al, an increased risk of hypoglycemia can also occur in septic conditions (Odds Ratio (OR) 6.4 95% CI 2.3-17.3), malignancy (OR 2.6 95% IK 1.1-6.2), and decreased albumin levels (OR 4.3 95% IK 2.9-6.5) [13].…”

“…33 Several issues need to be taken into account when considering pharmacological treatment of pre-LT DM, such as the development of side effects, the risk of hypoglycaemia, and potential limitations due to the presence of acute kidney injury. 35 Moreover, specific guidelines for the use of antihyperglycaemic drugs are lacking, mainly because patients with cirrhosis are excluded from clinical trials, 36 leaving insulin as the only completely safe treatment in cirrhotic patients regardless of the stage of liver disease. Oral agents can be used with caution in patients with Child-Pugh class A or B.…”
