Consensus Statement: Academy of Nutrition and Dietetics and American Society for Parenteral and Enteral Nutrition

Abstract: The Academy of Nutrition and Dietetics (Academy) and the American Society for Parenteral and Enteral Nutrition (A.S.P.E.N.) recommend that a standardized set of diagnostic characteristics be used to identify and document adult malnutrition in routine clinical practice. An etiologically based diagnostic nomenclature that incorporates a current understanding of the role of the inflammatory response on malnutrition's incidence, progression, and resolution is proposed. Universal use of a single set of diagnostic c… Show more

“…MNT, including intensive dietary counseling, is the intervention that has been demonstrated to improve outcomes in head and neck and gastrointestinal cancer patients undergoing radiotherapy treatment (39)(40)(41)(42). Only 1 of the studies (35) used a validated global assessment tool to determine nutritional status as is recommended by current guidelines (43,44). The remaining studies used weight or body composition changes estimated from anthropometrical measurements to detect changes in nutritional status.…”

“…Comprehensive assessment of nutritional status by trained professionals, such as dietitians, is essential once malnutrition risk has been identified. Nutrition assessment uses a combination of medical history, nutritional data, physical examination, anthropometric measurements, and biochemical parameters to determine the individual's nutritional status (38).…”

“…It is imperative to have a consistent approach when defining malnutrition in the RAC setting as this will prevent the issue of potential misdiagnosis and facilitate improved correlation between effective nutritional interventions and health-related outcomes thereby influencing advocacy for the development and implementation of suitable policies, regulations and legislations (38). Residents found to be at risk of malnutrition should undergo comprehensive nutrition assessment by a dietitian or physician.…”

“…In outpatients, only for artificially fed patients, prealbumin was the best predictor of prognosis together with comorbidities (16). Dieticians regularly use prealbumin levels in assessing the nutritional needs of hospitalized patients especially the critically ill. Prealbumin is currently recognized as a faithful marker of malnutrition being introduced in practice guidelines (17). One of the most important aspects about prealbumin is the fact that its variations in time are more valuable than the absolute values (2).…”
