Consensus practice parameter: audiological assessment and management of unilateral hearing loss in children

Abstract: Objective: Provide recommendations to audiologists for the management of children with unilateral hearing loss (UHL) and for needed research that can lend further insight into important unanswered questions. Design: An international panel of experts on children with UHL was convened following a day and a half of presentations on the same. The evidence reviewed for this parameter was gathered through webbased literature searches specifically designed for academic and health care resources, recent systematic rev… Show more

“…These systematic reviews indicate that, despite the increased number of publications on children with UHL, it is difficult to draw any firm conclusions regarding the outcomes or options open to them due to the limited availability of quality studies. The consensus practice parameter for the audiological assessment and management of children with UHL has identified a number of key areas where consistency in study design and reporting could address the current knowledge gaps about outcomes and management options for children identified with UHL [10].…”

“…This consensus practice paper [10] has provided guidance on management and rehabilitative options for babies identified with UHL. Four areas for the management of hearing loss were discussedaudiological monitoring, developmental monitoring, early childhood intervention services, and hearing assistive technology management.…”

“…A solid view on which children with UCHL benefit from interventions is needed (45,46). Objective tests demonstrate that some children with UCHL improve their directional hearing while others hardly benefit or even deteriorate (9, 33, 47).…”

“…Currently, the Brazilian Unified Health System (UHS) provides for individuals with UHL, the conventional hearing aid and the Contralateral Routing of Signal (CROS) adaptation system (10) .Conventional hearing aid is poorly recommended by otorhinolaryngologists, as they offer few benefits to speech amplification, especially if the level of UHL is profound. Even for severe hearing losses, this device does not enable speech recognition in noise and improvement in SL due to the large asymmetry between the sides (11) . The CROS system has the advantage of speech detection on the side with UHL in silent situations and a better understanding of speech in noise, when speech is transmitted predominantly on the side without hearing loss.…”

“…However, this device does not allow for improvement in the SL, an ability that these individuals present the most difficulty (12) . Probably, for this reason, the device is often not well accepted, not to mention the aesthetic issue related to the stigma of hearing loss due to the need to use two hearing aids, although the loss is only on one side (11,12) .…”
