Introduction: The record of the activities performed by the health professional should compose the patient’s medical record, whose fundamental role is communication. One way to make registration easier is to create semi-structured models. Objective: To evaluate the applicability and reproducibility of a semi-structured model for the medication therapy management record in hospitals. Methods: A convenience sampling of fifteen clinical pharmacists working in public hospitals in Minas Gerais was used. The participants received the semi-structured model for medical record and detailed report of two simulated clinical cases. The records made by each participant were compared to a standard record - template and the verification items were evaluated and categorized into: 1- compliant, 2- partially compliant, and 3- non-compliant. Participants also answered an electronic questionnaire with questions related to clinical experience and training. The main variable evaluated was the compliance percentage, considering the total of items evaluated. And for all variables under study, absolute and relative frequency were determined. The influence of the participants’ individual characteristics was univariate analyzed. Results: Most of the group (58%) had been graduated in Pharmacy for over ten years and most (83%) reported having been trained in the clinical area. The characteristics of the participants did not affect the results obtained. The overall compliance percentage showed a good performance of pharmacists using this model (76%) and most participants considered the model to be highly applicable / useful (75%). Conclusion: The high percentage of compliance achieved demonstrates that the semi-structured medical record model developed in this study was considered applicable by most participants, who also had a good performance in its use, and may be a starting point for other pharmacists and services to develop their own models, adapted to the reality of each service.

BackgroundIn Latin America, clinical pharmacy has undergone significant changes in recent decades, with more pharmacists seeking advanced training. Clinical pharmacists are now recognized for their crucial role in patient care, particularly in medication management, patient education, and interprofessional collaboration. However, the region's progress in this field has been influenced by diverse economic, social, and cultural factors, leading to different levels of advancement across different countries.AimsTo describe the practice and training of professionals in clinical pharmacy, as a distinct field within the pharmaceutical profession, across eight Latin American countries.Materials & MethodsArgentina, Brazil, Chile, Colombia, Costa Rica, Mexico, Peru, and Uruguay were chosen for this review. Data on clinical pharmacy practices in these countries was collected through a survey comprising five sections with 21 targeted questions. Following the survey's completion, an extensive literature review was conducted across multiple databases.ResultsBrazil has the most responsive education system to meet the demands of the population for the pharmacy career, with a University to Pharmacy (UP) index of 0.31, the lowest among all the countries included. Brazil also offers the highest number of postgraduate programs in the area of clinical pharmacy. Furthermore, diverse basic and advanced clinical pharmacy practices were identified in the included regions. These practices range from pharmacotherapeutic monitoring of patients to interdisciplinary collaboration in managing critically ill hospitalized patients. Each country exhibits a varying degree of acceptance towards clinical pharmacy.ConclusionThe evolution of clinical pharmacy in Latin America offers a promising opportunity to optimize drug therapy and improve patient safety and care quality. However, challenges remain, such as the need for more clinical sub‐specializations and better recognition of pharmacists' roles within healthcare teams. To address these challenges, continued professional development and advocacy are essential to enhance clinical pharmacy practice in the region.

Este trabalho teve como objetivo identificar estudos que utilizaram o mnemônico FASTHUG-MAIDENS e suas adaptações na prática clínica. Foi conduzida uma pesquisa em bases de dados, além de busca manual por outras fontes, utilizando os termos "fasthug-maidens", "farmacêutico" e "farmácia", no período compreendido entre 1º de janeiro de 2011 e 9 de outubro de 2022. Estudos completos que descreveram a utilização do FASTHUG-MAIDENS e suas adaptações pelo farmacêutico clínico em qualquer unidade hospitalar foram incluídos. Foram identificados 112 registros nas bases pesquisadas, dos quais 8 foram incluídos. A maioria dos estudos foram conduzidos no Brasil, publicados entre os anos de 2021 e 2022 e destinados exclusivamente a pacientes críticos. Quatro estudos usaram o FASTHUG-MAIDENS como ferramenta para identificação de oportunidades de otimização da farmacoterapia. No geral, eles mostraram que o mnemônico ajudou o farmacêutico a identificar mais problemas relacionados aos medicamentos e realizar mais intervenções farmacêuticas junto à equipe interdisciplinar. Os outros quatro estudos utilizaram o FASTHUG-MAIDENS como base para a construção de uma nova ferramenta adaptada ao perfil dos pacientes e/ou realidade do serviço, para garantir maior direcionamento e eficiência das ações do farmacêutico. Pesquisadores e farmacêuticos devem ser encorajados a publicar os achados do uso desta ferramenta.
