Dental caries is a multifactor disease affecting a significant number of people throughout the world. However, in recent decades the widespread availability of fluoride and other preventive measures have resulted in a decline in the prevalence of caries among children and young adults. Currently, it is accepted that most carious dental lesions are restricted to specific anatomical sites. The aim of this chapter is to review the influence of dental anatomy on dental caries development while taking into account recent findings in cariology. Occlusal fissures in the first permanent molar are generally the first sites in the permanent dentition to develop caries. An increased risk of caries is also found in proximal contacting surfaces between two adjacent teeth. Moreover, a partially erupted tooth, which does not participate in mastication, is also at risk for caries since it may provide a more favorable environment for bacterial accumulation than a fully erupted tooth. Bacterial biofilm on the tooth is frequently a high risk caries environment. Understanding anatomical dental features is of great importance for guiding oral health hygiene and preventive measures. Finally, the development of dental disorders plays an important role in dental caries risk.

Background: In most dental schools, dental anatomy is taught in both theoretical and practical sessions during the first years of the Bachelor of Dental Surgery (BDS). This comprehensive course equips students with essential knowledge about dental morphology and occlusion. Furthermore, it serves as the fundamental basis for various clinical disciplines within dentistry, particularly those related to tooth restoration. Aim: The study aimed to evaluate dental students’ response to tooth carving sessions using wax blocks within the BDS curriculum at the Faculty of Dentistry in Benghazi and to assess the benefit of these sessions in improving their knowledge, and understanding of tooth morphology. Methods: A three-month cross-sectional descriptive study was conducted in 2022 among 474 undergraduate dental students at the Faculty of Dentistry in Benghazi. We employed a convenience sampling approach to collect the data. The questionnaire was prepared using a semi-structured proforma, which included questions about participants’ perceptions of tooth carving. Participants received a Google Form link through the telegram channel of the students. Descriptive statistics, including mean, standard deviation, percentage, and frequency were calculated using the SPSS 21. Results: A total of 474 dental students participated in this study. Among them 94% practiced tooth carving using wax blocks during their practical sessions of dental anatomy. More than 70% of participants agreed that tooth carving enhanced their understanding of precise tooth morphology, dental anatomy, and proper occlusion relationships. Interestingly, 63% of the participants believed that live demonstration assisted with step-by-step video tutorials would be more effective in practicing tooth carving. Conclusion: Based on the results, it is evident that most students at the Dental Faculty of Benghazi recognize the significant importance of tooth carving in their educational journey as future dental practitioners. This valuable data can inform the development of dental education programs, ensuring continued emphasis on teaching tooth carving during the undergraduate stage. However, as we move forward, exploring innovative approaches becomes essential. Incorporating newer techniques such as computer-aided platforms and video tutorials could enhance student engagement and provide a deeper understanding of tooth morphology and anatomy.

Knowledge of dental anatomy is of great importance in the practice of dentistry, especially in oral rehabilitation, because without this knowledge, professional practice is not possible. Dental carving plays a major role in training dental students as it develops their manual dexterity. This randomized controlled trial aimed to evaluate the influence of didactic-theoretical reinforcement on the theoretical and practical knowledge of dental anatomy of preclinical students by examining the quality of the anatomical restorations performed by these students before and after a didactic-theoretical reinforcement. For the evaluation of theoretical knowledge, a questionnaire with closed questions about dental anatomy was used. To evaluate the effect of didactic reinforcement on dental carvings, two groups of 15 preclinical students were assessed. Experimental group (G1) received a three-hour theoretical tutoring on dental anatomy, while the control group (G2) did not. The dental carving scores obtained by the two different groups were compared using Student's t-test. Cohen's d was used to estimate the effect sizes between groups. The frequency of correct answers given for each theoretical knowledge question was compared in each group using Fisher's exact test. T-test was also used to compare the means of the two groups' final scores of theoretical evaluations. To compare these final scores obtained in both carving and theoretical tests, a principal component analysis was performed with different items assessed in each test to obtain factor loading scores and a final weighted score, where factor loadings were considered for each item. Weighted scores were compared using t-test. Also, scores obtained during the head and neck course were assessed and compared using t-test. Spearman's correlation test was used to assess the correlation between scores obtained prior to the anatomy course and scores obtained in the dental carving exercise. The theoretical evaluation revealed no significant difference between the grades (mean ± SD) of G1 (85.1 ± 6.6%) and G2 (86.2 ± 9.1%) with the grades of a baseline test that was previously obtained when students submitted to the study (P = 0.725). Regarding the tooth carving assessment, the dental carving quality by students of G1 has significantly improved, except for tooth #23 (P = 0.096). Theoretical reinforcement of dental anatomy seems to improve the students' carving performance but does not enhance their knowledge about dental anatomy.
