В последние годы предметом исследований клиницистов стали механизмы развития диастолической дисфункции миокарда левого желудочка и ее влияние на течение ишемической болезни сердца. Интерес к данной проблеме в настоящее время обусловлен увеличением объемов и сложности выполняемых опе-раций на открытом сердце. В доступной литературе очень мало данных о синхронности восстановления про цессов расслабления и сокращения миокарда после реваскуляризации. В связи с этим актуальным пред ставляется оценить изменение диастолической функции левого желудочка после коронарного шун-тирования и установить ее возможную взаимосвязь с динамикой систолической функции.Ключевые слова: диастолическая функция, ишемическая болезнь сердца, реваскуляризация. In recent years, clinicians have started to study the mechanisms in development of diastolic dysfunction of the left ventricle and its influence on the course of coronary artery disease. This is a consequence of increasing number and complexity open heart surgery. In the literature, few data about simultaneous recovery processes of relaxation and contractility after revascularization. In this regard, actual to assess diastolic function of the left ventricle after coronary artery bypass graft surgery and to identify a possible connection with changes in systolic function.Keywords: diastolic function, ischemic heart disease, revascularization.Диастолическая функция (ДФ) левого желу-дочка представляет собой совокупность процес-сов расслабления и растяжения, происходящих в диастолу и обеспечивающих диастолическое наполнение левого желудочка (ЛЖ). Диастола длится от момента закрытия аортального клапана до закрытия митрального клапана. Ряд авторов под диастолической функцией понимает способ-ность желудочков к наполнению, которая зависит от двух основных факторов -активного расслаб-ления (релаксации) и пассивных диастолических свойств желудочка, определяющих его способ-ность к растяжению. Последние, в свою очередь, зависят от толщины миокарда, степени его фи-брозирования, инфильтрации и физиологическо-го состояния [1]. Поступление крови в желудочек осуществляется в три фазы -фазу быстрого на-полнения в раннюю диастолу, фазу медленного (пассивного) наполнения и фазу активного напол-нения в конце диастолы во время систолы пред-сердий. Роль предсердий в условиях нормосисто-лии и при отсутствии нарушений диастолического расслабления относительно невелика: вклад си-столы предсердий в сердечный выброс не превы-шает 25%, что позволяет считать ее лишь допол-нительным насосом для наполнения желудочков, однако при прогрессировании хронической сер-дечной недостаточности систола предсердий при-обретает принципиально важную компенсатор-ную роль.Ранняя диастола наступает с периода изоволю-мического расслабления желудочка и приводит к выравниванию давления между предсердием и желудочком, что, в свою очередь, зависит от дав-ления в самом предсердии и скорости расслабле-ния миокарда желудочка. Previously the subject of much debate, there is now consensus that diastolic heart failure (DHF) represents a distinct form of heart failure. Epidemiologic data indicate that DHF is common. Indeed, there is evidence that, among elderly persons, DHF is more common than systolic heart failure (SHF). Like SHF, DHF is associated with significant morbidity, mortality, and cost; however, few clinical trials focusing on isolated DHF have been completed. Much of the treatment of DHF is based on current concepts of the pathophysiology of DHF, small clinical studies, and experience gained from treating patients with SHF. The diagnosis of DHF is clinical; data supporting the establishment of a diagnosis of DHF are limited. Differences exist in prognosis and treatment between diastolic and systolic heart failure. This article reviews diastolic heart failure with emphasis on evidence-based management, aimed at primary care physicians who routinely provide care to patients with DHF.
