Congenital rubella infection after previous immunity of the mother

Abstract: A newborn boy was admitted with a congenital rubella infection. Seven years previously his mother had been vaccinated against rubella; 3 years previously rubella immunity had been confirmed. Therefore, intrauterine transmission must have occurred after maternal reinfection during pregnancy. Prenatal diagnosis of rubella embryopathy with serological methods after subclinical maternal reinfection is nearly impossible.

“…On another hand, RV serology based on the use of the RV-EIA does not reliably distinguish between protective and nonprotective antibodies. Several cases of congenital rubella after reinfection during pregnancy have been reported during the last decade (7,8,11,20). In the course of this study, we have identified patients unable to raise an antibody response following RV vaccination.…”

“…It is well established that immunity acquired following natural RV infection remains longer than the vaccine-induced immunity (12). In both instances however, effective immunity is not always established, as numerous cases of RV reinfections have been documented (2,7,8,11,12,17,20).…”

“…Morgan-Capner [62], reviewing this problem in detail, analysed 10 previously published cases: in his opinion 3 vaccinated women who developed intrauterine infection were probably reinfected; 3 of the others, all with rashes, probably had primary rubella and the rest were difficult to interpret. Six further reports [60,[63][64][65][66][67] have added 11 well-documented cases (1 with a rash) leading to fetal infection (3 abortions, 8 badly damaged babies). One of these reports lays down serological and historical criteria for diagnosing reinfection [60].…”
