Congenital Pseudarthrosis of the Radius Treated With Gradual Distraction and Free Vascularized Fibular Graft: Case Report

“…In the case of congenital pseudarthrosis of the radius, cast immobilization, 1,2,4,7,10,[17][18][19] corticocancellous grafting with or without internal fixation, 3,5,6,20,21 intramedullary nailing, 6,8,22 electrical stimulation, 7,11 and cross-union of forearm bones 14,19,23 have been used with unpredictable results.…”

“…However, 8 cases of pseudarthrosis of long bones associated with NF1 and treated with cast immobilization failed to unite. 1,4,7,10,[17][18][19] Corticocancellous grafting with or without internal fixation has not produced adequate union rates. 3,5,6,8,10,17,19 -21,38,39 A review of the literature has shown that among 17 cases treated with corticocancellous grafting with or without internal fixation, only 10 cases of congenital pseudarthrosis of either or both forearm bones eventually united.…”

“…14 Many treatment modalities and different surgical procedures have been described for the treatment of congenital pseudarthrosis of the radius. Cast immobilization, 1,2,4,7,10,[17][18][19] corticocancellous autologous grafting, 3,5,6,20,21 intramedullary nailing, 6,8,22 electrical stimulation, 7,11 and cross-union of forearm bones 14,19,23 have not reproducibly achieved bony union. 14,15 Free vascularized fibular graft has been reported to be used with great success in the management of difficult orthopedic problems since 1975.…”

Primary Congenital Pseudoarthrosis of the Femur

“…3 As previously reported, conventional graft will not work in the majority of pseudoarthrosis cases secondary to neurofibromatosis, but needs vascularized graft and multiple surgical interventions. 2,4,5 In our case, a biopsy specimen for histopathology was not taken because the two femoral bone segments were already consolidated in normal-appearing bone. The pseudoarthrosis in our case appears to be primary and not secondary to local pathology, as postsurgery follow-up did not reveal nonunion or recurrence.…”
