Congenital chylothorax in a patient with 21 trisomy syndrome

Abstract: A female infant with 21 trisomy syndrome associated with congenital chylothorax was reported. She was born at a gestational age of 34 weeks by Cesarean section because of fetal hydrothorax and hydrops fetus, confirmed by ultrasonography at 32 weeks. Emergent resuscitation and immediate thoracentesis were performed soon after birth. After beginning breast feeding, the serous pleural fluid became opalescent and a diagnosis of congenital chylothorax was made. Feeding was changed to medium‐chain triglyceride (MCT)… Show more

“…Several cases have been reported about congenital or fetal pleural effusion with chromosomal anomaly such as Down syndrome and Turner syndrome (8)(9)(10)(11). Most of these pleural effusion were chylothorax or associated hydrops fetalis (6,9). Hence, karyotyping is indicated in a fetus or newborn with isolated pleural effusion for the evaluation of associated chromosomal anomaly (8,11).…”

“…There was no evidence of hydrops in either case and one had a significant cardiac defect. Subsequently, several cases of trisomy 21 have been reported with bilateral chylothoraces and non-immune hydrops detected antenatally in the absence of cardiac defects (Rodeck et al, 1988;Ilagan et al, 1992;Ho et al, 1989;Yamamoto et al, 1996).…”

“…Since then, based on a Medline search, a total of 11 cases of congenital pleural eusion associated with trisomy 21 have been reported in the English literature, 9 of which are in the form of congenital chylothorax [1,2,3,4,5,6,7]. In this report we present a further case of congenital pleural eusion associated with trisomy 21 which was also caused by chylothorax as in the majority of the other cases.…”
