- Các khối u nguyên phát ở tim cực kỳ hiếm gặp và chỉ chiếm khoảng 0,71% tổng số các khối u ở tim, trong số các khối u này, u cơ vân ở tim còn hiếm hơn. Các u này thường được phát hiện trong những năm đầu đời khi đánh giá van hoặc tắc nghẽn dòng máu. Biểu hiện lâm sàng tùy thuộc vào vị trí, số lượng và kích thước u. U có thể gây ra rối loạn huyết động nghiêm trọng như tắc nghẽn đường ra thất trái hoặc gây rối loạn nhịp tim. Hơn 90 % u cơ vân đa ổ có dấu hiệu của phức hợp bệnh xơ củ. Phẫu thuật cắt bỏ khối u là phương pháp điều trị hiệu quả khi khối u lớn gây ra các biến chứng huyết động nặng hoặc rối loạn nhịp tim tái phát và kháng trị. Chẩn đoán mô bệnh học là chẩn đoán quyết định. Việc phát hiện đột biến dòng mầm trên 2 gen TSC1 và TSC2 có liên quan đến sự hình thành phức hợp xơ củ, góp phần chẩn đoán u cơ vân đa ổ ở tim, bệnh xơ củ và tư vấn di truyền cho gia đình bệnh nhân.
- Báo cáo ca: Bệnh nhân nam hơn 1 tháng tuổi được chẩn đoán u tim, tắc nghẽn đường ra thất trái bằng siêu âm tim và MRI tim. Bệnh nhân được phẫu thuật tim, và các khối u (bên dưới van động mạch chủ và ở bờ tiểu nhĩ phải) đã được cắt bỏ. Xét nghiệm mô bệnh học và hóa mô miễn dịch khẳng định là các u cơ vân. Bệnh nhân hồi phục tốt sau phẫu thuật. Ngoài ra, xét nghiệm sinh học phân tử cho thấy một đột biến trong gen TSC2.

Abstract
- Background: Primary cardiac tumors are extremely rare and account for only about 0.71% of all cardiac tumours, among these tumours, the cardiac rhabdomyomas are even rarer. They are commonly discovered in the first years of life during an evaluation for a valve or other flow obstruction. The clinical features depend on the site, number and dimensions of the tumors. The tumors can cause serious hemodynamic disturbances as obstruct the left ventricular outflow tract or arrhythmia. More than 90 % multiple cardiac rhabdomyomas have signs of tuberous sclerosis complex. The surgical resection is indicated when large tumours cause severe haemodynamic complications or recurrent resistant arrhythmias. The histopathological diagnosis is the decisive diagnosis. The detection of germline mutations on 2 genes TSC1 and TSC2 are related to the formation of tuberous fibrous complex, contributing to the diagnosis of multiple cardiac rhabdomyoma, tuberous sclerosis and genetic counseling for the patient’s family.
- Case presentation: An over 1-month-old boy was diagnosed with cardiac tumors, left ventricular outflow tract obstruction by echocardiography and cardiac MRI. He undergoes heart surgery, and the tumors (below the aortic valve and in the right auricle) were removed. Histopathological and immunohistochemical examination confirmed the rhabdomyomas. The patient recovered well after surgery. In addition, molecular biology testing revealed a mutation in theTSC2 gene.
- Conclusions: We present this case to share our findings with the colleagues. Although a rarely reported case, we hope this cardiac rhabdomyoma case report and literature review can increase cardiac tumor awareness.
