ObjectivesPostoperative delirium (POD) is a common complication after elective cardiac surgery. Recent evidence indicates that a disruption in the normal activity of the cholinergic system may be associated with delirium.DesignProspective observational study.SettingSingle-centre at a European academic hospital.Primary and secondary outcome measuresIn our study the enzyme activities of acetylcholinesterase (AChE) and butyrylcholinesterase (BChE) were determined preoperatively as well as on the first and second postoperative day. The confusion assessment method for the intensive care unit was used to screen patients for the presence of POD.ResultsA total of 114 patients were included in the study. POD was associated with a decrease in BChE activity on postoperative day 1 (p=0.03). In addition, patients who developed POD, had significantly lower preoperative AChE activity than patients without POD (p<0.01). Multivariate analysis identified a preoperatively decreased AChE activity (OR 3.1; 95% CI 1.14 to 8.46), anticholinergic treatment (OR 5.09; 95% CI 1.51 to 17.23), elevated European System for Cardiac Operative Risk Evaluation (OR 3.68; 95% CI 1.04 to 12.99) and age (OR 3.02; 95% CI 1.06 to 8.62) to be independently associated with the development of POD.ConclusionsWe conclude that a reduction in the acetylcholine hydrolysing enzyme activity in patients undergoing cardiac surgery may correlate with the development of POD.

Introduction: Clinical research has suggested that there might be a correlation between postoperative delirium (POD) and obstructive sleep apnea (OSA) syndrome. We aimed to assess the association between POD and OSA syndrome. Methods: The electronic database PubMed was searched using combinations of terms for “Delirium,'’ “Obstructive Sleep Apnea,'’ and “postoperative delirium.'’ Excluded were studies without comparison as well as cross-sectional studies, case series, and case reports. The search was conducted with restriction toward English or not to the date of publication. Results: We included four studies in this review. In two of those studies, an association was found between POD and OSA syndrome. Pooled analysis showed a significant correlation between the two. Conclusion: High-quality studies regarding the subject are rare and heterogeneous. However, despite the lack of high-quality studies regarding the subject, the ones that are performed well conclude that there is a correlation between POD and OSA syndrome. Future studies addressing the matter should be well set up controlled clinical trials to draw conclusions and be able to investigate modifiable factors that can be used in a standardized protocol.

Delirium is a common presentation after acute stroke. Post-stroke delirium is related to poor recovery, higher rates of mortality, falls and longer hospital stays. Delirium can lead to challenging behaviour, such as anger, aggression and confusion. Therefore, it is important to promptly identify delirium for early management and to reduce the negative impact on post-stroke recovery and outcomes. An important aspect of identifying delirium depends on the use of efficient, easy to use and validated assessment tools. A wide range of tools are available, although it is not known how accurately they can identify post-stroke delirium. This article critically appraises a systematic review that identified delirium screening tools for patients with acute stroke.
