Evaluation of Upper Airway in Children with Obstructive Sleep Apnea Using Cone‐Beam Computed Tomography

Objective Cone‐beam computed tomography (CBCT) offers three‐dimensional structures in assessing upper airway of patients. This study aims to compare the cone‐beam computerized tomography scan measurements between children with obstructive sleep apnea (OSA) and primary snoring. Study Design Case–control study. Methods This prospective study was conducted in a tertiary referral center. Thirty‐six children with moderate‐to‐severe OSA (with apnea‐hypopnea index [AHI] > 5 events/hour) and 36 age‐, gender‐, and obesity‐matched children with primary snoring (AHI <1) were enrolled. The measurements in CBCT parameters were compared between children with moderate‐to‐severe OSA and primary snorers by conditional logistic regression model. Results A total of 72 children (mean age, 7.9 ± 2.8 years; 64% male) were included. Children with moderate‐to‐severe OSA had a significantly smaller nasopharyngeal (2900 ± 1400 vs. 3800 ± 1800 mm3, P = .017) and oropharyngeal airway volume (5600 ± 2700 vs. 7400 ± 4000 mm3, P = .026) than those with primary snoring. Children with moderate‐to‐severe OSA, as compared to primary snorers, also had a significantly smaller minimal airway area in nasopharynx (77.4 ± 37.7 vs. 107.7 ± 52.0 mm2, P = .006) and oropharynx (66.6 ± 61.9 vs. 101.6 ± 65.8 mm2, P = .023). Moreover, the airway length was not significantly different between children with moderate‐to‐severe OSA and primary snoring. Conclusions The three‐dimensional CBCT airway analysis could be used as a useful tool to evaluate upper airway in children with OSA. Level of Evidence 3 Laryngoscope, 131:680–685, 2021

La expansión del maxilar ya sea rápida o lenta, ha sido un procedimiento utilizado durante años para la corrección de maloclusiones transversales. Sus beneficios y efectos a nivel dentario, como de sutura palatina media son conocidos. Sin embargo, en los últimos años la repercusión y sus efectos a nivel de vía aérea han sido estudiados y se han establecido como un punto de interés sobre todo durante el tratamiento de pacientes con problemas respiratorios. Según la bibliografía actual, la expansión rápida del maxilar induce un aumento significativo del volumen de la vía aérea superior. Hoy en día, con el alto desarrollo de técnicas basadas en imágenes 3D y con el advenimiento de la tomografía computarizada Cone Beam, es posible realizar análisis cuantitativos y evaluar los efectos de la expansión maxilar a nivel de la vía aérea superior en los tres sentidos del espacio. Mediante el uso de imagenología es posible visualizar la ganancia en la vía aérea superior pero aún no es posible evidenciar si este aumento se correlaciona con una mejora en la función respiratoria.

Objectives To investigate the associations between nasal airway volume and the presence and severity of obstructive sleep apnea (OSA) in adults. Materials and Methods The medical records of adult patients who visited the sleep clinic at University Hospital between June 2013 and April 2017 and underwent overnight polysomnography for the diagnosis of obstructive sleep apnea were reviewed retrospectively. Using computed tomography, the volumes of the nasal airways and maxillary sinuses were measured, and associations with the presence and severity of OSA were analyzed while controlling for the effects of possible confounders such as lateral cephalometric variables, maxillary widths, tongue/hyoid position, and soft palate dimensions. Results Comparison between normal subjects and patients with OSA revealed that the latter had decreased ratios of maxillary sinus volume to whole nasal airway volume (P = .029) than normal subjects. OSA severity was greater in those with inferior positions of the hyoid (P = .010), in older patients (P = .011), and in those with high body mass index (P = .001). The volume of the total nasal airway or maxillary sinuses were not associated with OSA severity. Conclusions A decreased ratio of maxillary sinus volume to whole nasal airway volume is associated with adult OSA. However, OSA severity is not associated with either maxillary sinus volume or whole nasal airway volume.
