ObjectivesHIV prevalence among Cambodian female sex workers (FSW) is among the highest in Southeast Asia. We describe HIV prevalence and associated risk exposures in FSW sampled serially in Phnom Penh, Cambodia (Young Women's Health Study (YWHS)), before and after the implementation of a new law designed to combat human trafficking and sexual exploitation.DesignCross-sectional analysis of baseline data from two prospective cohorts.SettingCommunity-based study in Phnom Penh, Cambodia.ParticipantsWomen aged 15–29 years, reporting ≥2 sexual partners in the last month and/or engaged in transactional sex in the last 3 months, were enrolled in the studies in 2007 (N=161; YWHS-1), and 2009 (N=220; YWHS-2) following information sessions where 285 and 345 women attended.Primary outcomesHIV prevalence, sexual risk behaviour, amphetamine-type stimulant (ATS) and alcohol use, and work-related factors were compared in the two groups, enrolled before and after implementation of the new law.ResultsParticipants in the two cohorts were similar in age (median 25 years), but YWHS-2 women reported fewer sex partners, more alcohol use and less ATS use. A higher proportion of YWHS-2 compared with YWHS-1 women worked in entertainment-based venues (68% vs 31%, respectively). HIV prevalence was significantly lower in the more recently sampled women: 9.2% (95% CI 4.5% to 13.8%) vs 23% (95% CI 16.5% to 29.7%).ConclusionsSex work context and risk have shifted among young FSW in Phnom Penh, following implementation of anti-prostitution and anti-trafficking laws. While both cohorts were recruited using the same eligibility criteria, more recently sampled women had lower prevalence of sexual risk and HIV infection. Women engaging more directly in transactional sex have become harder to sample and access. Future prevention research and programmes need to consider how new policies and demographic changes in FSW impact HIV transmission.

Sexual Infidelity in China: Prevalence and Gender-Specific Correlates

The nature of extra-relational sex in societies with rapidly changing sexual mores and widespread commercial sex remains under-explored. The 2006 Sexuality Survey of China provides a national probability survey with data on 3,567 people 18-49 years old who were in a marital (89%) or dating/cohabiting (11%) relationship. In attitudes, extramarital sex was completely unacceptable to 74% of women and 60% of men and either somewhat or completely unacceptable to 95% of women and men. Most (77%) women wanted severe punishment of men's short-term commercial sex and women's jealousy was equally elevated by their primary partner's episodes of commercial and non-commercial sex. Nevertheless, the prevalence of infidelity during the last 12 months was 4.5% (women's non-commercial sex), 11.0% (men's non-commercial), and 5.5% (men's commercial), with each percent matching or exceeding the median for other countries. In multivariate equations for non-commercial infidelity, men's infidelity was significantly more responsive to sexual dissatisfaction with his primary partner while women's was more responsive to deficits in love. In commercial sex, men were uninfluenced by primary partner deficits in love, sexual satisfaction or oral sex-pursuing, it would seem, simply a greater variety of sexual partners. In a "trading up" pattern, women partnered with low income men had elevated infidelity. The minority of women reporting early masturbation and premarital sex were just as likely as men with these backgrounds to have elevated infidelity. The Chinese patterns provide ample material for deliberations on gender similarities and differences in extra-relational sex.

Women engaged in sex work bear a disproportionate burden of HIV infection worldwide, particularly in low to middle income countries. Stakeholders interested in promoting prevention and treatment programs are challenged to efficiently and effectively target heterogeneous groups of women. This problem is particularly difficult because it is nearly impossible to know how those groups are composed a priori. Although grouping based on individual variables (e.g., age or place of solicitation) can describe a sample of women engaged in sex work, selecting these variables requires a strong intuitive understanding of the population. Furthermore, this approach is difficult to quantify and has the potential to reinforce preconceived notions, rather than generate new information. We aimed to investigate groupings of women engaged in sex work. The data were collected from a sample of 204 women who were referred to an HIV prevention intervention in Ulaanbaatar, Mongolia. Latent Class Analysis was used to create subgroups of women engaged in sex work, based on personal and financial risk factors. This analysis found three latent classes, representing unique response pattern profiles of personal and financial risk. The current study approached typology research in a novel, more empirical way and provided a description of different subgroups, which may respond differently to HIV risk interventions.
