The Coronavirus disease (COVID-19) pandemic is disrupting our health environment. As expected, studies highlighted the great susceptibility of cancer patients to COVID-19 and more severe complications, leading oncologists to deeply rethink patient cancer care.This review is dedicated to the optimization of care pathways and therapeutics in cancer patients during the pandemic and aims to discuss successive issues.First we focused on the international guidelines proposing adjustments and alternative options to cancer care in order to limit hospital admission and cytopenic treatment in cancer patients, most of whom are immunocompromised.In addition cancer patients are prone to polypharmacy, enhancing the risk of drug-related problems as adverse events and drug-drug interactions. A unidade de terapia intensiva (UTI) é o local no ambiente hospitalar onde as chances de Interação medicamentosa (IM) são maiores. Os medicamentos utilizados, para reconstrução terapêutica do paciente oncológico na UTI, podem apresentar maior interação com os antineoplásicos. A inclusão do farmacêutico na equipe de saúde se torna relevante no sentido de colaborar na qualidade assistencial aos pacientes dessa unidade. Objetivou-se descrever a importância do farmacêutico clínico na diminuição das interações medicamentosas ao paciente oncológico na unidade de terapia intensiva. Realizou-se uma revisão de literatura utilizando periódicos indexados nas bases de dados Scientific Electronic Library Online-Scielo (Scielo), British Pharmacological Society e Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS). Incluindo no estudo, periódicos nacionais e internacionais publicados no período de 2013 a 2018. Foi evidenciado que devido ao grande número de medicamentos, o risco de IM nos pacientes oncológicos são altos, pois podem afetar o monitoramento plasmático de drogas. Várias classes de medicamentos podem interagir com quimioterápicos, um exemplo é a interação do metotrexato com um antiinflamatório não esteroidal (AINE), pois sua administração conjunta pode ocasionar uma obstrução do canal de excreção da droga antineoplásica. Os erros de prescrição podem levar a essas IM, porém podem ser evitadas com as intervenções farmacêuticas (IF), seja durante a visita à beira do leito, durante a discussão de casos, na avaliação da prescrição ou na análise de prontuários. Sendo assim, o farmacêutico clínico desempenha uma função fundamental junto a equipe de saúde na UTI, nos ajustes posológicos afim de evitar as IM, e contribuir para melhoria da qualidade de vida do paciente oncológico.

The present study aims to list the oral antineoplastic agents (OAAs) most dispensed in an oncology hospital, identifying their doses, indications, contraindications and possible drug interactions (DIs). Documentary, retrospective, analytical and descriptive study, where OAAs dispensation spreadsheets were analyzed, from january to july 2019. Data were obtained from Micromedex® and analyzed by SPSS® (version 20) using descriptive statistics. 22 types of OAAs were dispensed, making a total of 360.703 drugs. Anastrozole was the most prescribed (51,85%) and hormone therapy agents (32%) were considered the main group. Tyrosine kinase inhibitors exposed the highest rates of contraindications (n=64). With regard to serious risk interactions, nilotinib (n=264), dasatinib (n=182) and metotrexate (n=134) were the drugs with the highest number of reports. The OAAs that did not present any type of interaction were anastrozole and diethylstilbestrol. Of the pharmacological agents that most interacted with OAAs, live vaccines (n=11) were the most present in contraindications and warfarin was the one that had the highest rate of serious interactions (n=8). As for the repercussions of DIs, the most frequent indices were the increased risk of prolongation of the QT interval (n=67) and increased exposure to a drug (n=51). Bearing in mind that cancer therapy requires special attention because of the possible interactions with OAAs, these results are shown to be a useful tool for the entire multiprofessional health team, especially the pharmacist, who has an essential role in the pharmaceutical assistance service on an outpatient basis.
