Wearable medical technologies (wearables) are a type of digital health technology that have the potential to transform the delivery of healthcare by offering a personalized, predictive, and preventative form of patient care. Unfortunately, the adoption of these technologies remains low, indicating an issue with the innovation ecosystem. Existing literature on innovation ecosystems fails to address wearables and the tradeoffs between profitability and health outcomes, hindering discussion on how to promote the widespread adoption of wearables. This paper reviews and synthesizes literature from the business community and healthcare technology community on innovation ecosystems to contribute towards developing a more robust model of the innovation ecosystem for developing wearables. Future work aims to enrich this conceptualization with interviews of key actors in the innovation ecosystem.Clinical Relevance-This establishes the relevance of innovation ecosystems in enabling the usage and uptake of digital health technologies that meet patient needs.

Background: South Africa’s public healthcare sector is overburdened, especially its under-resourced primary healthcare delivery system. This burden could be relieved by alleviating the population’s ill-health, focusing on the social determinants of health. These include living conditions and levels of social cohesion. In an attempt to address the aforementioned ‘challenge landscape’, this article considers socio-economic empowerment of those marginalised members of society living at the base of the pyramid (BOP) to improve factors contributing to poor health. We propose that Innovation Platforms (IPs) offer opportunities to achieve this by drawing diverse stakeholders together, which should include marginalised individuals, to pool resources and knowledge and collaborate around a specific set of challenges. Method: A Grounded Theory approach is utilised to develop the framework comprising concept definition from a systemized literature review. It is evaluated through various progressive stages through three phases of evaluation: 1) the initial framework was subjected to scrutiny in a theoretical case study, 2) a first-pass semi-structured interview and later four more semi-structured interviews with subject matter experts, and 3) an instrumental case study to refine the framework and to understand its application in a particular situation (this included four stakeholder interviews and a workshop and feedback session with the project champion). Results: This article contributes to the extant literature by addressing the lack of guidance on stakeholder engagement practices critical to the proper functioning of IPs in the context of overcoming the complex challenges associated with social determinants of health. The final output of the study is a refined management tool for stakeholder engagement in IPs. The tool provides practical recommendations to support policy makers, researchers and practitioners in 1) establishing IPs, 2) identifying areas for improvement and 3) identifying reasons for an IP’s failure and lessons to learn.

South Africa’s interventions to address complex social challenges rely on coordination across several sectors and between different levels of government and society. Improved alignment, planning and coordination are needed when addressing the causal factors of these social challenges. These causal factors include the environments in which people live and their behaviours. Furthermore, emphasis is placed on the recurring engagement of civil society, especially of marginalized stakeholders, as participants in the efforts to address the challenges. The study draws from the promise shown by stakeholder networks, termed Innovation Platforms, in other Sub-Saharan Africa countries to address such complex social challenges. The study aimed to improve the understanding of how a stakeholder network’s engagement practices impact the effectiveness of the network. To this end, a conceptual framework and management tool for stakeholder engagement in IPs is proposed. The study followed the conceptual framework analysis procedure to develop, evaluate and refine the conceptual framework. The article describes the core research outcomes of the framework development approach, starting with a systematized literature review to identify core concepts, followed by interviews with experts and a case study to refine the framework content. The case study applied the framework to develop recommendations for improved engagement in a stakeholder network which has been established around the challenge of vagrancy in Stellenbosch, South Africa. The result of the approach is a multidimensional framework for conceptualizing stakeholder engagement practices in a variety of contexts. The focus of the framework content remains on the practices of engagement which enable effective and fruitful stakeholder interactions within and around a network. The study delivered valuable insights into the nature of some development initiatives in South Africa and the impact of stakeholder engagement on them.
