Conceptual Design of a Universal Donor Screening Approach for Vaginal Microbiota Transplant

Abstract: The success of fecal microbiota transplant (FMT) in treating recurrent Clostridioides difficile infection has led to growing excitement about the potential of using transplanted human material as a therapy for a wide range of diseases and conditions related to microbial dysbiosis. We anticipate that the next frontier of microbiota transplantation will be vaginal microbiota transplant (VMT). The composition of the vaginal microbiota has broad impact on sexual and reproductive health. The … Show more

“…The authors suggested a list of tests to perform on donor candidates before VMT, such as HIV (1 and 2), HAV, HBV, HCV, EBV, Treponema pallidum, Chlamydia trachomatis, Neisseria gonorrhoeae, Toxoplasma gondii, and others. According to their criteria, from 20 candidates enrolled in the study, only seven were eligible for vaginal fluid donations, indicating the importance of donor screening prior to VMT [155].…”

“…In our experience, the IRB at Johns Hopkins viewed the issue similarly, and required communication with the FDA prior to considering study applications. Thus, we (LME, KD, FZ, and collaborators) have worked with the FDA to establish a framework for screening potential CVS donors, 73 as well as procedures for handling, storing, and performing quality control checks on CVS. Based on our experience, we describe here some of the issues that may arise for researchers in fulfilling the IND requirements.…”

“…could be the causes of disease suppression. One of the earliest documented transfers of suppressive microbiota was published by Menzies [112]. In this study, a small amount of transferred living, conducive, sterilized soil resulted in a dramatic reduction in potato scab caused by Streptomyces scabies.…”
