Recent studies on major depression (MD) have used noninvasive brain stimulation techniques such as transcranial direct current stimulation (tDCS) to improve impaired emotion and cognition in MD. However, such experiments have yielded mixed results, specifically with respect to cognition in MD. This study aimed to investigate whether anodal and cathodal tDCS applied over the dorsolateral prefrontal cortex (DLPFC) would significantly improve visual working memory and reduce depressive symptoms in patients with MD. Thirty patients with major depression (n = 30) were randomly assigned to receive either experimental (active) or control (sham) tDCS. To measure cognitive functions, the participants underwent a series of visual memory neuropsychological tasks; and to measure depression symptoms, the Beck Depression Inventory (BDI) and Hamilton Depression Scale (HDRS) were used. The parameters of active tDCS included 2 mA for 20 min per day for 10 consecutive days, anode over the left DLPFC (F3), cathode over the right DLPFC (F4) region. After 10 sessions of anodal and cathodal tDCS, patients showed significantly improved performance in visual working memory tasks. The same results were observed for depression symptoms. This study showed that anodal tDCS over left DLPFC, concurrently with cathodal tDCS over right DLPFC, improved cognitive impairment (specifically visual working memory), as well as reduced depressive symptoms in patients with MD. This finding provides evidence that supports effectiveness of a specific montage of tDCS to improve impaired cognition in MD, specifically in visual working memory.

Background. Interest in the neuro-cognitive profile of patients with schizophrenia and co-morbid obsessive compulsive disorder (schizo-OCD) is rising in response to reports of high co-morbidity rates. Whereas schizophrenia has been associated with global impairment in a wide range of neuro-cognitive domains, OCD is associated with specific deficits featuring impaired performance on tasks of motor and cognitive inhibition involving frontostriatal neurocircuitry.Method. We compared cognitive function using the CANTAB battery in patients with schizo-OCD (n=12) and a schizophrenia group without OCD symptoms (n=16). The groups were matched for IQ, gender, age, medication, and duration of illness.Results. The schizo-OCD patients made significantly more errors on a task of attentional set-shifting (ID-ED set-shift task). By contrast, no significant differences emerged on the Stockings of Cambridge task, the Cambridge Gamble Task or the Affective Go/NoGo tasks. No correlation emerged between ID-ED performance and severity of schizophrenia, OCD or depressive symptoms, consistent with neurocognitive impairment holding trait rather than statemarker status. Schizo-obsessives also exhibited a trend toward more motor tics emphasizing a neurological contribution to the disorder. Conclusion.Our findings reveal a more severe attentional set-shifting deficit and neurological abnormality that may be fundamental to the neuro-cognitive profile of schizo-OCD. The clinical implications of these impairments merit further exploration in larger studies.

Computerized neuropsychological tests can be effective and have many benefits. This paper addresses these issues using the computer-based Cambridge Neuropsychological Test Automated Battery (CANTAB) to assess Brazilian children and adults. Preliminary normative data for Brazilian children obtained in a series of CANTAB subtests are presented together with data from adults (controls) and samples of patients with multiple sclerosis and Duchenne muscular dystrophy. The results obtained in children confirmed the normative values reported for American children (Luciana & Nelson, 2002), indicating high applicability of the CANTAB battery for children in the evaluation of visual memory, working memory, planning, and set shifting. For multiple sclerosis patients, CANTAB revealed differences between the control and multiple sclerosis groups for all subtests employed, including visual memory, working memory, spatial memory, decision making, inhibitory control, and sustained attention. In children with Duchenne muscular dystrophy, the CANTAB tests revealed specific detriments in attention and memory. These results of the application of CANTAB in different patient groups indicate that the battery is appropriate for assessing several central nervous system diseases.
