Computer tomography guided lung biopsy using interactive breath-hold control: a randomized study

Abstract: The use of IBC for CT-guided lung biopsy was not an advantage for unselected patients in our care, since it did not improve the biopsy accuracy and the risk of pneumothorax was increased.

“…Pneumothorax is a common and potentially life-threatening complication of puncturing the thorax. Emphysema has been widely recognized as an independent risk factor of pneumothorax after biopsy; however, other risk factors, such as lesion size, lesion depth, and patient age remain controversial (15,16,(21)(22)(23)(24)(25)(26). Although most researchers believe that the size and depth of the lesion as well as patient age are risk factors, there is no published evidence definitively supporting this.…”

“…18 Conversely, the level of breath holding at the end of expiration was not reproducible in another study. 17 We hypothesized that this is because patients tend to breathe deeply or perform Valsalva when asked to hold their breath, 4 which might induce significant movement of the nodule during full inspiration and at the end of exhalation. 19 Conversely, in patients with a large tumor or tumor in upper lobe, it was possible to conduct biopsies using free breathing without respiratory targeting.…”

“…All the methods need to manually set the learning rate or use a common method of artificially attenuating the learning rate to reduce the learning rate according to the number of training iteration steps. The optimization method is an adaptive optimization method that can adjust the learning rate according to each parameter [ 17 ]. First, for the number of iteration steps f and the weight item λ x , assume that there is a variable f t , x =∇ λ S ( λ x ), which is represented by f t for the convenience of presentation.…”
