Aims Transient ischemic dilation (TID) is a marker of underlying extensive coronary artery disease (CAD) during myocardial perfusion imaging (MPI). The cut off for a normal TID ratio (TIDr) value is often derived from a cohort of individuals with no apparent CAD. Varying criteria have been used to define the absence of CAD. We aim to derive TIDr cut offs using patients with normal MPI and coronary artery calcium (CAC) score, of zero and compare TIDr obtained from different software packages. Methods and results We studied 232 patients with zero CAC and normal MPI undergoing exercise or dipyridamole stress using either a 1 or 2-day protocol. All patients were scanned in the supine position with a cadmium-zinc-telluride camera. TIDr was automatically generated using Quantitative Perfusion SPECT (QPS) software initially, and subsequently using Myometrix for comparison. The TIDr cut offs calculated using the mean + 2 standard deviation were 1.29 and 1.24 for the 1 and 2-day protocol groups respectively. Dipyridamole stress resulted in significantly higher mean TIDr in the 2-day protocol group (1.07 ± 0.13 vs 1.01 ± 0.12, p = 0.035). Myometrix derived TIDr were also significantly lower compared to QPS derived values for most protocols except 2-day exercise stress. Conclusion This study is the first to derive TIDr threshold values using a normal population defined by zero CAC and normal MPI. TIDr also varied depending on stress modality, protocol as well as software used.

This prospective randomized study assessed myocardial perfusion imaging with the high-sensitivity D.SPECT cadmium-zinc-telluride camera in a forward-leaning bikerlike position, which may potentially lower diaphragmatic attenuation and reduce breathing-related cardiac motion, in a manner comparable to the prone position proposed with other SPECT cameras. Methods: Patients referred for a stress-rest 99m Tc-sestamibi protocol and positioned in the biker position, with the chest leaning forward on the D.SPECT camera-head at 35°from vertical, had an additional resting D.SPECT recording in the supine position (n 5 40) or in the sitting position with the back rearward at 30°from vertical (n 5 40). Segments with attenuation artifacts were defined as those with less than 65% uptake but with strictly normal contractility at gated SPECT and no defect reversibility from stress images. Results: The biker position was associated with lower heart-to-detector distances than the supine or sitting positions (both P , 0.001); lower cardiac motion amplitudes, assessed on panograms, than the supine position (P , 0.001); and fewer segments with attenuation artifacts than the supine position (on average, 1.10 ± 1.01 vs. 1.90 ± 1.74, P 5 0.010) or the sitting position (0.75 ± 0.93 vs. 1.38 ± 1.60, P 5 0.011). Conclusion: Myocardial perfusion images from D.SPECT are enhanced for patients positioned in a forward-leaning bikerlike position comparatively to sitting or supine positions, with a notably lower rate of attenuation artifacts.

Abstract: Increased upright TID measurements on a CZT camera are associated with extensive CAD. The upright TID measurements can serve in an adjunctive role to SSS, and may be most effective in patients with a normal upright exercise LVEF.
