Chronic lower back pain is a potentially incapacitating condition associated with disc degeneration. Although therapy is primarily pharmaceutical, surgery comprising arthrodesis constitutes an alternative. Anterior intersomatic lumbar arthrodesis (ALIF, anterior interbody lumbar fusion) is the reference approach, although total disc arthroplasty may also be undertaken. Analysis of pelvic and spinal parameters provides the best indication of sagittal balance. Materials and methods This was a prospective study in a continuous series of 99 patients presenting chronic lower back pain due to disc disease. Pelvic incidence, sacral slope, pelvic tilt, spino-sacral angle (SSA) and the four back types in the Roussouly classification were studied in radiographs of the whole spine under load using an EOS imaging system. Results The pre-operative SSA value for the study population was 126.09°± 8.45°and the mean spine tilt angle was 90°compared with 95°in healthy subjects. Following surgery, the SSA was considerably increased in the discal arthroplasty, resulting in a significantly more balanced spinal position. In the group of patients undergoing arthrodesis using the ALIF technique, no such significant improvement was found despite the use of a lordosis cage. We showed that in cases of low pelvic incidence, it was necessary to maintain a Roussouly type 1 or 2 back without increasing lordosis. The results demonstrated the value of L4-L5 disc prostheses in these subjects. L5-S1 arthrodesis seemed a more suitable approach for treating patients with elevated sacral slope (back type 3 or 4). This new type of analysis of sagittal parameters should be performed prior to all surgical procedures involving lumbar prostheses.

Nowadays, conventional or digitalized teleradiography remains the most commonly used tool for the study of the sagittal balance, sometimes with secondary digitalization. The irradiation given by this technique is important and the photographic results are often poor. Some radiographic tables allow the realization of digitalized spinal radiographs by simultaneous translation of X-ray tube and receptor. EOS system is a new, very low dose system which gives good quality images, permits a simultaneous acquisition of upright frontal and sagittal views, is able to cover in the same time the spine and the lower limbs and study the axial plane on 3D envelope reconstructions. In the future, this low dose system should take a great place in the study of the pelvispinal balance. On the lateral view, several pelvic (incidence, pelvic tilt, sacral slope) and spinal (lumbar lordosis, thoracic kyphosis, Th9 sagittal offset, C7 plumb line) parameters are drawn to define the pelvispinal balance. All are interdependent. Pelvic incidence is an individual anatomic characteristic that corresponds to the ''thickness'' of the pelvis and governs the spinal balance. Pelvis and spine, in a harmonious whole, can be compared to an accordion, more or less compressed or stretched.

IntroductionThe treatment of most spinal diseases is a difficult task because of the rate of immediate, mid-and long-term complications and failures [16]. Surgical planning and clinical follow up often rely on postural and/or dynamic X-ray exams, among other imaging options.Many authors investigated the radiological parameters liable to rule patient's evolution, such as postural and balance parameters [2,4,8,9,11,12,26]. These parameters might be classified in pelvic parameters, either morphologic (incidence) or positional (sacral slope [4,11,12], pelvic angle [8,9]), and spinal parameters (such as lordosis, kyphosis [4,11,12] or plumblines [8,9]). Their measurement is tedious and time consuming while the accuracy of the results may be highly variable depending on the method employed. Furthermore, when trying to compare the results between studies, a major difficulty comes from the differences in the definition, identification and measurement of parameters.Recent computer-based tools allowed for a rapid progress in quantitative measurements of a large range of parameters [9,17,19,20,23,26]. Moreover, these tools allow easy standardisation of X-ray films measurements, which is of paramount importance when comparing studies in multicentric analysis. However, validation of these tools is essential [3,17,18].In 1999, the team of LBM and LIO developed an Xray films analysis software (SpineView 1.0) allowing for the measurement of the main lumbar dynamic and sagittal postural parameters of the spine. The evaluation of this first software was achieved by comparison with direct measurements on plain X-ray films [17,18], and showed good reliability of the measurements. Abstract The objective of this study was to evaluate an X-ray films analysis software, i.e. to estimate the reliability and validity of clinical measurements by means of this software. The authors first performed tests of precision and reproducibility of measures. The precision for dynamic modules was estimated at ±2°for the lumbar analysis and ±3°for the cervical one. Mean reproducibility coefficients calculated for postural modules are about 4°for the angular parameters and 3 mm for the linear ones. We also evaluated clinical applicability of the software through its validity. Reference values calculated on a population of healthy subjects showed agreement with the literature. Then, when analysing postural X-ray films of severe scoliotic patients, we found that inter-observer reproducibility coefficients show a lower reliability of measurements; the main cause seems to be the low visibility of anatomic landmarks due to the quality of X-ray films and to the degree of deformity.
