Computer Algorithms in the Search for Unrelated Stem Cell Donors

Abstract: Hematopoietic stem cell transplantation (HSCT) is a medical procedure in the field of hematology and oncology, most often performed for patients with certain cancers of the blood or bone marrow. A lot of patients have no suitable HLA-matched donor within their family, so physicians must activate a “donor search process” by interacting with national and international donor registries who will search their databases for adult unrelated donors or cord blood units (CBU). Information and communication technologies … Show more

“…Therefore, the rapid and reliable identification of suitable adult volunteer HSC donor candidates and/or cord blood units for individual patients is of primary importance. This challenging search process is routinely performed in a donor registry or cord blood bank by a computer program in which the HLA‐matching algorithm (HMA) can be regarded as the core element . For simplicity, we will use the term ‘donor’ to refer to donors of HSCs from bone marrow or peripheral blood and cord blood units and the term ‘donor registry’ shall include cord blood banks.…”

“…Typically, a prospective BMT recipient's siblings are screened for suitability of donors and failing this, the search is widened to unrelated persons. Nevertheless, at the time of this writing, a large percentage of patients requiring allogeneic HSC are unable to identify matching donors .…”

“…Esta busca por um doador compatível é realizada através de um algoritmo de compatibilidade HLA (HLA matching algorithm -HMA) [Bochtler et al 2016], cujo objetivo é buscar em um cadastro de voluntários os potenciais doadores [Steiner 2012]. A compatibilidade entre paciente-doador é definida pelo número de alelos compartilhados pelos genes HLA-A, -B, -C, -DRB1 e -DQB1 [Geffard et al 2019].…”
