Computed Tomography in Primary Reticulum Cell Sarcoma of the Brain

Abstract: The CT findings in 16 patients with histologically proved primary central nervous system (CNS) reticulum cell sarcoma (RCS) showed this malignancy to have predilection for the basal ganglia and thalamus, the periventricular white matter, the corpus callosum, and the vermis cerebelli. In 43% of patients with untreated tumor it presented as multifocal lesions; in each of these patients the basal ganglia were involved in conjuction with another site. In the other 57% of patients with untreated tumor presenting as… Show more

“…1). The CT findings were characteristic, as emphasized by Enzmann, et aL 6 The mass seen on plain CT scans was usually isodense or slightly hyperdense and was surrounded by an irregular lowdensity area. Marked enhancement was always observed following the intravenous administration of contrast medium (Fig.…”

“…Three tumours were of mixed hypodense and isodense attenuation, two were uniformly hypodense and one mixed hypo/hyperdense. The majority, 18 altogether, were ) Spillante, Kendall, Moselej" hyperdense before contrast enhancement. One large, dense and uniformly enhancing lesion had developed a low attenuation centre when the CT examination was repeated after two weeks because of marked deterioration in the patient's condition (fig 4).…”

“…[16][17][18] It therefore seemed important to determine whether a survey of the clinical features and CT appearances might enable one to improve on this, not least because "the most remarkable clinical feature of the tumours is their response to radiotherapy".' Many are multiple5 19 20 and so readily confused with metastases; because of the results of this confusion, in particular, it seemed important to decide whether any clinical or radiographic pattern would enable the clinician to recognise the possibility of this diagnosis and so encourage biopsy even in the seemingly "hopeless" case.…”

“…Although cytologic examinationof vitreous aspirate wasreported to be useful for the diagnosis of malignant lymphomain the opthalmological field (3,9), it is good to always consider that uveitis can be an early manifestation of primary ocular and CNS malignant lymphoma, and to carefully follow-up the neurological examination of these patients. For the diagnosis of primary CNS malignant lymphoma, an increased B2-microglobulin level in CSF (ll) and neuroradiological findings on CT (12,13) or MRI (14) have been reported to be useful. Actually, the increased level of B2-microglobulin in CSF and marked enhancement by contrast enhanced material on CT scans and by Gd-DTPAon MRscans were observed in the present case.…”
