Computed Tomography Angiography for the Chimeric Anterolateral Thigh Flap in the Reconstruction of the Upper Extremity

Abstract: The chimeric anterolateral thigh (ALT) flap has been an ideal option for restoring complex soft tissue defects. However, as the vascular anatomy of the ALT is variable, the positions of its perforators are uncertain, which makes the surgery more challenging. Therefore, our study utilized computed tomographic angiography (CTA) for preoperative perforator mapping. From October 2013 to October 2015, 37 patients suffering upper extremity soft tissue defects underwent CTA before ALT flap reconstruction. Perforators… Show more

“…In our study, we performed computed tomography angiography (CTA) or color Doppler sonography (CDS) to map perforators before applying an ALT perforator flap to repair soft tissue defects. According to our previous study [16], CTA had a higher sensitivity than CDS for detecting vessels. Therefore, we typically used CTA first to create clear 3D images of the vessels and surrounding structures, as well as to mark the skin.…”

“…Identification of multiple perforators preoperatively is critical in cases of complex reconstructions requiring multiple independent skin paddles or for maximizing flap size. 31,32 Moreover, when the recipient site requires a long pedicle, preoperative CTA allows for the choice of a system with maximal length from LCFA takeoff to the most distal perforator supplying the ALT flap. This can similarly be useful in the setting of a distally based ALT flap wherein proximal thigh flap design and reach to the knee and tibia are essential.…”

“…By contrast, C-CTA provided a satisfactory mapping efficiency, which was higher than that of T-CTA and any previously reported values. 6,8,12,19,22 Notably, by using the C-CTA technique, we report promising results for demonstrating the presence of perforators, especially the most used perforators (type B) and the most difficultto-detect perforators (type C). 6 More importantly, C-CTA could accurately predict the origin and course if the perforator was detected successfully.…”
