Comprehensive morphologic and functional imaging of heart transplant patients: first experience with dynamic perfusion CT

Abstract: • Stress CT perfusion imaging enables the detection of myocardial ischemia • CT myocardial perfusion imaging can be combined with coronary computed tomography angiography • Combining perfusion and coronary CT imaging is accurate in heart transplant patients • CT myocardial perfusion imaging can be performed at a reasonable radiation dose.

“… 45) Stress perfusion CT may allow for the detection of perfusion change in LV myocardium of patients with CAV. 46) Finally, the coronary artery calcium (CAC) score seems to be of particular interest as recent prospective study including 113 transplant recipients cited an overall sensitivity of 88% and negative predictive value of 97% for CAC in detecting CAV 2–3 (moderate-severe CAV). 47) …”

“…Therefore, MPI could be valuable in selected cases. Moreover, emerging CT technologies including CT-derived fractional flow reserve and CT MPI offer the potential to functionally assess the coronary arteries without using additional modalities [21,22]. Further investigations of these methods are needed to confirm this in HTx patients.…”

“…In a study by Oebel et al, a combined protocol of CCTA plus stress CTP was tested to unmask both anatomical and functional CAV-related consequences, and revealed a very high accuracy on a per-patient basis when compared to ICA and stress-CMR [138].…”
