Comprehensive management of locally advanced breast cancer

Abstract: The definition of locally advanced breast cancer includes patients with large tumors, extensive regional lymph node involvement, or direct involvement of the skin or underlying chest wall. Most of these patients have very poor survival with standard treatment modalities, and over the past 20 years combined modality therapy has been used to improve local and systemic control. There is limited information suggesting that patients with operable locally advanced breast cancer have an improved survival if treated w… Show more

“…Neoadjuvant chemotherapy is now the preferred approach for treating patients with inflammatory breast cancer and/or locally advanced breast carcinoma (LABC) [1,2]. There are multiple advantages with this approach, including the downstaging of an inoperable cancer to an operable one, the increase of the availability of breast conservation for those patients otherwise required to undergo a mastectomy, and the provision of an in vivo testing of the efficacy of reduction in the primary tumor volume during treatment, a surrogate marker for a reduction of micrometastatic disease [3,4].…”

“…Despite aggressive local therapy, long-term patient survival is still poor. LABC remains controversial because of uncertainties in determining the optimal intensity and duration of neoadjuvant chemotherapy and evaluating therapeutic response (2,4,5).…”

“…It can also be used as a method for evaluating the response of breast carcinoma to chemotherapy treatment, thereby providing an in vivo indication of the chemosensitivity of the tumor [45]. Tiling et al [46] showed that PET and breast scintigraphy with 99m…”
