“Comprehensive Care Model” for Diabetes Within a Primary Care Clinic: Pre-Visit Planning, at-Visit Efficiency, and Inter-Visit Care

Abstract: BackgroundUPP-GIM, a large urban academic primary-care practice and a patient-centered medical home, treats approximately 1800 diabetics.
