Comprehensive Analysis of Adverse Events Associated With Per Oral Endoscopic Myotomy in 1826 Patients: An International Multicenter Study

Abstract: This large study comprehensively assessed the safety of POEM and highly suggests POEM as a relatively safe procedure when performed by experts at tertiary centers with an overall 7.5% prevalence of AEs. Severe AEs are rare. Sigmoid-type esophagus, endoscopist experience, type of knife, and current used can be considered as predictive factors of AE occurrence.

“…The PDS was shown to correlate well with procedural velocity and is an accurate measure of procedural difficulty. It has previously been shown that operative time indirectly correlates with complications . This supports the theory that the PDS will also correlate with complication rate.…”

“…A multicenter comprehensive analysis of 1826 patients reported mild, moderate, and severe adverse events occurring with frequencies of 6.4%, 1.7%, and 0.5%, respectively. 47 One of the greatest concerns after POEM is reflux, evidence of which is observed in up to 58% of patients (Table 2). 42 …”

“…One report suggests a doubling of adverse events with POEM in patients with sigmoid esophagus. 59 Table 3 outlines distinguishing treatment considerations among the achalasia syndromes, along with preferred treatment options and rationale for each. Note that the suggestions made are based on expert opinion, given that no existing treatment trial has ever stratified patients to this degree.…”

“…The technique of POET is based on the tunneling concept developed for POEM, in which the endoscopic resection occurs in the submucosal space while maintaining the integrity of the overlying mucosa. POEM is now performed worldwide and has been shown to be safe and effective without any severe adverse events or postoperative strictures . Our study also demonstrates that the technique of POET, like POEM, is also safe and effective.…”
